| 目的探究血清前蛋白转化酶枯草杆菌蛋白酶 /Kexin 6型( PCSK6)、半乳糖凝集素 -9(Galectin-9)和可溶性肾素(原)受体[s(P)RR]在慢性心力衰竭( CHF)病情和预后评估中的价值。方法选取 2021年 12月至 2022年 12月于河南黄河科技学院附属医院接受治疗的 CHF病人( n=93)作为 CHF组,另选取同期在该院体检健康者( n=93)作为健康组。采用酶联免疫吸附分析( ELISA)测定血清 PCSK6、Galectin-9、s(P)RR表达水平;采用受试者操作特征曲线( ROC曲线)分析血清 PCSK6、Galectin-9、 s(P)RR对 CHF病人病情及预后评估的价值;影响 CHF预后的因素采用多因素 logistic回归分析。结果与健康组相比, CHF组血清 PCSK6、Galectin-9、s(P)RR表达水平较高[( 52.94±6.58)μg/L比( 38.01±5.21)μg/L、(50.80±7.54)pg/L比( 24.04±4.43)pg/ L、(26.28±5.31)ng/L比( 18.04±3.23)ng/L](P<0.001)。 Ⅱ、Ⅲ、Ⅳ级 CHF病人血清 PCSK6、Galectin-9、s(P)RR表达水平依次显著升高[( 46.51±6.25)μg/L比( 54.29±6.62)μg/L比( 62.44±7.16)μg/L、(44.04±7.22)pg/L比( 53.28±7.51)pg/L比( 58.19±8.27)pg/L(22.25±4.46)ng/L比( 27.54±5.59)ng/L比( 31.23±6.31)ng/L](P<0.001)。与非主要心血管不良事件( MACE)组( n=61)相比, MACE组( n=32)血清 PCSK6、Galectin-9、s(P)RR表达水平较高[( 64.16±7.18)μg/L比( 47.05±6.26)μg/L、(58.12±8.16)pg/L比(46.96±7.22)pg/L、(31.12±5.84)ng/L比( 23.74±5.03)ng/L](P<0.001)。多因素 logistic回归分析显示, PCSK6、Galectin-9、s(P)RR是影响 CHF病人预后的危险因素(P<0.05)。血清 PCSK6、Galectin-9、s(P)RR三者联合预测 CHF病人预后、病情程度的曲线下面积(AUC)最高,优于血清 PCSK6、Galectin-9、s(P)RR各自单独预测(均 P<0.05)。结论 CHF病人血清 PCSK6、Galectin-9、s(P)RR表达水平显著升高,三者联合对病情及预后评估价值更好。 |

| Objective To explore the value of serum proprotein convertase subtilisin/Kexin type 6 (PCSK6), Galectin-9, and soluble (pro) renin receptor [s(P)RR] in evaluating the severity and prognosis of chronic heart failure (CHF).Methods CHF patients (n=93)who received treatment at He'nan Yellow River University of Science and Technology Affiliated Hospital from December 2021 to De.cember 2022 were collected as the CHF group, and healthy individuals (n=93) who underwent physical examinations at the same hospi.tal concurrently were regarded as the healthy control group. Enzyme linked immunosorbent assay (ELISA) was applied to measure theexpression levels of serum PCSK6, Galectin-9, and s(P)RR. The value of serum PCSK6, Galectin-9, and s(P)RR levels in the diagnosisand prognosis evaluation of CHF patients was analyzed by plotting receiver operating characteristic curve (ROC curve), and the factorsaffecting the prognosis of CHF were analyzed using multivariate logistic regression.Results Compared with the healthy control group, the expression levels of serum PCSK6, Galectin-9, and s(P)RR in the CHF group were significantly increased [(52.94±6.58) μg/L vs. (38.01±5.21) μg/L, (50.80±7.54) pg/L vs. (24.04±4.43) pg/L, (26.28±5.31) ng/L vs. (18.04±3.23) ng/L] (P<0.001). The expression levels of serum PCSK6, Galectin-9, and s(P)RR in patients with grade Ⅱ, Ⅲ, and Ⅳ CHF were significantly increased in sequence [(46.51± 6.25) μg/L vs. (54.29±6.62) μg/L vs. (62.44±7.16) μg/L, (44.04±7.22) pg/L vs. (53.28±7.51) pg/L vs. (58.19±8.27) pg/L, (22.25±4.46) ng/L vs. (27.54±5.59) ng/L vs. (31.23±6.31) ng/L] (P<0.001). Compared with the non major adverse cardiovascular events (MACE) group (n=61), the expression levels of serum PCSK6, Galectin-9, and s(P)RR in the MACE group (n=32) were increased [(64.16±7.18) μg/L vs. (47.05±6.26) μg/L, (58.12±8.16) pg/L vs. (46.96±7.22) pg/L, (31.12±5.84) ng/L vs. (23.74±5.03) ng/L] (P<0.001). Multivariate logistic regression analysis results showed that PCSK6, Galectin-9, and s(P)RR were risk factors affecting the prognosis of CHF patients (P<0.05). The combined prediction of serum PCSK6, Galectin-9, and s(P)RR for the prognosis and severity of CHF patients had thehighest area under the curve (AUC), which was better than the individual prediction of serum PCSK6, Galectin-9, and s(P)RR (all P< 0.05).Conclusion The expression levels of serum PCSK6, Galectin-9, and s(P)RR in CHF patients are significantly increased, andthe combination of them was more valuable in evaluating the severity and prognosis of CHF. |
