| 目的探讨宫颈癌病人血清微 RNA-628(miR-628)、微 RNA-410(miR-410)的水平及临床意义。方法以 2017年 12月至 2020年 8月在北京怀柔医院就诊的 100例病人作为宫颈癌组,另从该院选取同期就诊的 97例宫颈上皮内瘤变( CIN)病人作为 CIN组,再选取同期体检健康人员 91例作为健康组;再根据 3年生存情况将宫颈癌病人分为预后良好组( n=66)和预后不良组(n=34)。选择实时荧光定量逆转录聚合酶链反应( qRT-PCR)检测 miR-410、miR-628表达水平。通过 Cox回归分析对宫颈癌病人预后的影响因素进行探究,利用受试者操作特征曲线( ROC曲线)评估病人血清 miR-410、miR-628水平对宫颈癌病人 3年预后的诊断价值。结果健康组、 CIN组及宫颈癌组病人血清 miR-410(1.02±0.19、0.85±0.15、0.62±0.12)、 miR-628(1.01±0.18、0.86±0.16、0.64±0.11)水平比较,差异有统计学意义( P<0.001);与健康组比较, CIN组、宫颈癌组病人血清 miR-410、miR-628水平降低( P<0.05);与 CIN组比较,宫颈癌组病人血清 miR-410、miR-628水平较低( P<0.05)。临床分期 Ⅱa+Ⅱb期、浸润深度 >1/2肌层及有淋巴结转移的宫颈癌病人血清 miR-410、miR-628水平均显著低于临床分期 Ⅰ期、浸润深度 ≤1/2肌层及无淋巴结转移的宫颈癌病人( P<0.05)。与预后良好组比较,预后不良组血清 miR-410、miR-628水平均较低,且二者与临床分期及淋巴结转移有关( P<0.05)。 miR-410、miR-628水平为影响宫颈癌病人预后的独立保护因素( P<0.05)。血清 miR-410、miR-628及联合诊断宫颈癌病人预后不良的 ROC曲线下面积( AUC)及其 95%CI分别为 0.82(0.73,0.89)、 0.81(0.72,0.88)、 0.92(0.85,0.96),二者联合诊断效能高于单独检测效能(均 P<0.05)。结论宫颈癌病人血清 miR-410、miR-628表达水平较低,且与其临床病理特征及预后有关,可能成为宫颈癌诊断与预后评估的标志物,具有一定临床意义。 |

| Objective To explore the levels and clinical significance of serum microRNA-628 (miR-628) and microRNA-410 (miR-410) in patients with cervical cancer.Methods A hundred cervical cancer patients, who visited Beijing Huairou Hospital from Decem.ber 2017 to August 2020, were collected as the study subjects (cervical cancer group). Another 97 patients with cervical intraepithelialneoplasia (CIN) who visited the same hospital during the same period were chosen as the CIN group. Additionally, 91 healthy individu.als who underwent physical examinations during the same period were selected as the healthy control group. Cervical cancer patientswere assigned to good prognosis group (n=66) or poor prognosis group (n=34) based on their 3-year survival status. The real-time fluo. rescent quantitative PCR (qRT-PCR) method was selected to detect the expression levels of miR-410 and miR-628. The influencing fac.tors for the prognosis of cervical cancer patients were explored through Cox regression analysis. The receiver operating characteristiccurve (ROC curve) was used to evaluate the diagnostic value of serum miR-410 and miR-628 levels in predicting the 3-year prognosis of cervical cancer patients.Results There were statistically significant differences in the levels of serum miR-410 (1.02±0.19, 0.85± 0.15, 0.62±0.12) and miR-628 (1.01±0.18, 0.86±0.16, 0.64±0.11) among the healthy control group, the CIN group and the cervical can. cer group (P<0.001). Compared with the control group, the levels of serum miR-410 and miR-628 in the CIN group and cervical cancer group were significantly reduced (P<0.05); compared with the CIN group, the levels of serum miR-410 and miR-628 in the cervical can. cer group were significantly reduced (P<0.05). The levels of serum miR-410 and miR-628 in cervical cancer patients with clinical stageⅡa+Ⅱb, infiltration depth>1/2 muscle layer, and lymph node metastasis were significantly lower than those in cervical cancer patientswith clinical stage Ⅰ, infiltration depth≤1/2 muscle layer, and no lymph node metastasis (P<0.05). Compared with the good prognosis group, serum miR-410 and miR-628 levels were significantly lower in cervical cancer patients in the poor prognosis group, and bothwere associated with clinical stage and lymph node metastasis (P<0.05). miR-410 and miR-628 were recognized as independent protec. tive factors affecting the prognosis of patients with cervical cancer (P<0.05). The area under the ROC curve (AUC) and its 95%CI for predicting poor prognosis in cervical cancer patients using serum miR-410, miR-628, and their combination were 0.82 (0.73, 0.89), (0.72, 0.88), and 0.92 (0.85, 0.96), respectively, and the combined diagnostic efficacy of the two was higher than the individual de.tection efficacy (both P<0.05).Conclusions The expression levels of miR-410 and miR-628 in the serum of patients with cervical can.cer are relatively low, which are related to the clinical pathological characteristics and prognosis of the disease. They may serve asmarkers for the diagnosis and prognosis assessment of cervical cancer and have certain clinical significance. |
