      目的探讨急诊严重创伤病人血清爱帕琳肽 13(apelin-13)、和肽素的表达及其与病情及预后的关系。方法 151例研究对象(创伤组)来自华北医疗健康集团邢台总医院 2021年 6月至 2023年 6月收治的急诊严重创伤病人,收集同期 154例体检健康受试者作为健康组,酶联免疫吸附分析(ELISA)检测血清 apelin-13、和肽素水平;根据随访结果将病人分为预后良好组(n=126)和预后不良组(n=25);分析病人血清 apelin-13、和肽素与急性生理学和慢性健康状况评价 Ⅱ(APACHEⅡ)、ISS评分的相关性,影响病人预后的因素,及血清 apelin-13、和肽素对病人预后的预测价值。结果与健康组比较,创伤组 apelin-13水平降低[(10.85±1.24)μg/L比( 21.02±2.13)μg/L],和肽素水平较高[(410.60±50.25)μg/L比( 126.04±13.14)μg/L,P<0.05];预后不良组较预后良好组 APACHEⅡ评分、 ISS评分、和肽素、收缩压均升高, apelin-13和平均动脉压下降(P<0.05);病人 APACHEⅡ评分和 ISS评分与血清 apelin-13呈负相关,与血清和肽素呈正相关(P<0.05);血清 apelin-13、和肽素单独及联合预测急诊严重创伤病人预后不良的 AUC分别为 0.89、0.89、0.97。结论急诊严重创伤病人血清 apelin-13水平降低,和肽素水平升高,与病情严重和预后不良关系密切。

      Objective To explore the expression of serum apelin-13 and copeptin in patients with emergency severe trauma, and their relationship with the disease and prognosis.Methods A total of 151 study subjects (trauma group) were emergency severe trauma pa-tients accepted by Xingtai General Hospital of North China Medical and Health Group from June 2021 to June 2023, 154 healthy subjectswho underwent physical examinations were collected as the reference group. ELISA assay was applied to detect serum apelin-13 and co-peptin levels. According to the follow-up results, patients were separated into a good prognosis group (n=126) and a poor prognosis group (n=25). The correlation between serum apelin-13 and copeptin expression levels with APACHE Ⅱ and ISS scores, the factors affectingprognosis, and the predictive value of serum apelin-13 and copeptin for prognosis were analyzed.Results Compared with the healthy group, the level of apelin-13 in the trauma group was lower [(10.85 ± 1.24) μg/L vs. (21.02 ± 2.13) μg/L], and the level of copeptin was higher (410.60 ± 50.25) μg/L vs. (126.04 ± 13.14) μg/L, P < 0.05]. The poor prognosis group had higher systolic blood pressure, APACHEⅡ score, ISS score, copeptin, and systolic blood pressure, and lower apelin-13 and mean arterial pressure than the good prognosis group (P<0.05). The APACHE Ⅱ and ISS scores of patients were negatively correlated with serum apelin-13, and positively correlated with se-rum copeptin (P < 0.05). The AUC for predicting poor prognosis in emergency severe trauma patients with serum apelin-13 copeptin alone and in combination was 0.89, 0.89, and 0.97, respectively.Conclusion The serum apelin-13 level decreases and the copeptin level in-creases in patients with emergency severe trauma, both are closely related to the severity of the disease and poor prognosis.
