| 经动脉化疗栓塞术(TACE)是目前治疗不可切除肝细胞癌的重要方式,传统碘油化疗栓塞(cTACE)存在药物洗脱快、栓塞不均匀及系统毒性较高等缺陷,载药微球TACE(DEB-TACE)通过实现化疗药物的局部缓释和稳定栓塞可一定程度上改善了上述不足。尤其是小粒径载药微球(<300μm)因其可深入肿瘤末梢血管、增强组织渗透性并提高局部药物浓度而受到广泛关注。目前的研究表明小粒径载药微球疗效确切,总体安全性可控,但需注意胆道损伤及非靶栓塞风险,未来仍有待进一步优化适应证和粒径选择,以进一步提高疗效并保障安全。 |

| Transarterial chemoembolization (TACE) remains a cornerstone in the management of unresectable hepatocellular carcinoma (HCC). However, conventional TACE (cTACE) is frequently hindered by rapid systemic drug washout, heterogeneous embolization, and consequent systemic toxicity. Drug-eluting bead TACE (DEB-TACE) was introduced to overcome these limitations by providing sustained locoregional drug delivery coupled with durable arterial occlusion. In particular, small-caliber drug-eluting beads (< 300 μm) have garnered considerable interest due to their capacity to penetrate deeply into distal tumor-feeding arterioles, thereby improving spatial distribution and maximizing intratumoral drug concentrations. Current clinical evidence indicates that small-caliber DEBs yield robust oncological outcomes alongside a manageable safety profile. Nevertheless, their profound distal penetrance necessitates caution regarding the risks of ischemic biliary injury and non-target embolization. Future research should focus on refining clinical indications and standardizing particle size selection to strike an optimal balance between maximal antitumoral efficacy and procedural safety. |
