| 目的探讨维持性血液透析( MHD)病人血清 C1q/肿瘤坏死因子相关蛋白 6(CTRP6)、内皮细胞特异性分子 -1(ESM-1)水平与动静脉内瘘( AVF)狭窄的相关性。方法选取武警四川省总队医院 2020年 3月至 2023年 10月建立 AVF的 186例 MHD病人,根据是否发生狭窄分为 AVF狭窄组( n=58)和 AVF非狭窄组( n=128)。酶联免疫吸附分析( ELISA)检测病人血清 CTRP6、ESM-1水平。 Pearson相关性分析 AVF狭窄病人血清 CTRP6、ESM-1水平与血红蛋白( Hb)、 C反应蛋白( CRP)、血清铁蛋白( SF)的相关性;多因素 logistic回归分析探索影响 AVF狭窄的因素;受试者操作特征曲线( ROC曲线)分析血清 CTRP6、 ESM-1水平对 AVF狭窄的诊断价值。结果与 AVF非狭窄组相比, AVF狭窄组血清 CTRP6(168.54±54.72)ng/L、Hb(82.57±22.61)g/L水平降低, ESM-1(2.64±0.85)μg/L、CRP(130.54±30.67)mg/L、SF(2850.59±920.48)ng/L水平升高( P<0.05)。血清 CTRP6水平与 Hb呈正相关,与 CRP、SF呈负相关(P<0.05);血清 ESM-1水平与 Hb呈负相关,与 CRP、SF呈正相关(P<0.05)。血清 ESM-1、CRP、SF是影响 AVF狭窄的危险因素, CTRP6、Hb为保护因素( P<0.05)。 ROC曲线分析显示,血清 CTRP6、ESM-1及二者联合诊断诊断 AVF狭窄的曲线下面积( AUC)分别为 0.82、0.85及 0.91,二者联合诊断显著优于血清 CTRP6、ESM-1单独诊断( Z二者联合 -CTRP6=2.60、Z二者联合 -ESM-1=2.53,均 P<0.05)。结论 AVF狭窄 MHD病人血清 CTRP6水平降低, ESM-1水平升高;血清 CTRP6、ESM-1水平与 AVF狭窄密切相关,二者联合检测的诊断效能较高。 |

| Objective To explore the correlation between serum levels of C1q/tumor necrosis factor related protein 6 (CTRP6) and en-dothelial cell specific molecule-1 (ESM-1) and arteriovenous fistula (AVF) stenosis in maintenance hemodialysis (MHD) patients.Meth. ods One hundred and eighty-six MHD patients who established AVF in The Hospital of the Armed Police Force of Sichuan Provincefrom March 2020 to October 2023 were included and separated into AVF stenosis group (n=58) and AVF non stenosis group (n=128)based on whether stenosis occurred. Enzyme linked immunosorbent assay (ELISA) was applied to detect serum levels of CTRP6 andESM-1 in patients. Pearson correlation was applied to analyze the correlation between serum CTRP6, ESM-1 levels, hemoglobin (Hb), C-reactive protein (CRP), and serum ferritin (SF) in patients with AVF stenosis. Multivariate logistic regression was applied to analyzethe factors affecting AVF stenosis. Receiver operating characteristic curve (ROC curve) was applied to analyze the diagnostic value ofserum CTRP6 and ESM-1 levels for AVF stenosis.Results Compared with the AVF non-stenosis group, the serum levels of CTRP6(168.54±54.72) ng/L and Hb (82.57±22.61) g/L in the AVF stenosis group were decreased, while the levels of ESM-1 (2.64±0.85) μg/L, CRP (130.54±30.67) mg/L, and SF (2 850.59±920.48) ng/L were increased (P<0.05). According to Pearson correlation analysis, serumCTRP6 level was positively correlated with Hb and negatively correlated with CRP and SF (P<0.05); the serum ESM-1 level was nega-tively correlated with Hb and positively correlated with CRP and SF (P<0.05). Multivariate Logistic regression analysis showed that se-rum ESM-1, CRP, and SF were risk factors for AVF stenosis, while CTRP6 and Hb were protective factors (P<0.05). ROC curve analy-sis showed that the area under the curve (AUC) of serum CTRP6, ESM-1, and their combined diagnosis for AVF stenosis was 0.82,0.85, and 0.91, respectively, the combined diagnosis of the two was obviously better than the individual diagnosis of serum CTRP6 andESM-1(Z combination -CTRP6=2.60, Z combination -ESM-1=2.53, both P<0.05).Conclusions The serum CTRP6 level decreases and ESM-1 level in-creases in AVF stenosis MHD patients, which is closely related to AVF stenosis. The diagnostic efficacy of combined detection of thetwo is higher. |
