      目的分析血清中微 RNA-28(miR-28)和微 RNA-142(miR-142)水平,探讨其对急性缺血性脑卒中( AIS)病人预后的预测价值。方法以 2020年 12月至 2022年 12月于聊城市第二人民医院就诊的 86例 AIS病人( AIS组)为研究对象,另选取 100例健康体检者(健康组);实时荧光定量 PCR(qRT-PCR)法检测血清中 miR-28和 miR-142表达水平;利用 Spearman相关分析 AIS病人血清中 miR-28和 miR-142表达水平与美国国立卫生研究院卒中量表(NIHSS)评分、改良 Rankin量表(MRS)评分之间的相关性;采用 logistic回归分析影响 AIS病人预后的因素;采用受试者操作特征曲线(ROC曲线)分析血清中 miR-28和 miR-142表达水平对 AIS病人预后的预测价值。结果 AIS组病人血清中 miR-28(0.64±0.07比 1.01±0.10)和 miR-142(0.55±0.06比 1.02±0.07)表达水平均低于健康组( P<0.05);轻度卒中组、中度卒中组、重度卒中组 AIS病人血清中 miR-28(0.87±0.10、0.69±0.09、 0.48±0.08)和 miR-142(0.63±0.07、0.56±0.06、0.50±0.05)水平依次显著降低(P<0.05);预后不良组 AIS病人血清中 miR-28(0.55±0.10比 0.71±0.08)和 miR-142(0.50±0.05比 0.59±0.07)表达水平低于预后良好组( P<0.05)。 Spearman相关分析显示, AIS病人血清中 miR-28和 miR-142表达水平与 NIHSS评分、 MRS评分之间分别呈负相关( P<0.05)。与预后良好组相比,预后不良组高血压、糖尿病病人占比、收缩压、舒张压、低密度脂蛋白胆固醇明显较高(P<0.05); logistic回归分析结果显示, miR-28、miR-142、高血压、糖尿病病人占比和收缩压均为影响 AIS病人预后不良的因素(P<0.05)。 ROC曲线分析显示,血清中 miR-28和 miR-142表达水平联合预测 AIS预后较 miR-28或 miR-142单一指标预测效能更好。结论 AIS病人血清中 miR-28和 miR-142表达水平下降,与 AIS病情严重程度有关,两者联合具有较高的 AIS预后预测价值。

      Objective To analyze the levels of miR-28 and miR-142 in serum and to explore their predictive value in the prognosis of patients with acute ischemic stroke (AIS).Methods A total of 86 AIS patients (AIS group) who visited Liaocheng Second People'sHospital from December 2020 to December 2022 were regarded as the research subjects, and another 100 healthy examinees (controlgroup) were collected. Real-time fluorescence quantitative PCR (qRT-PCR) method was applied to detect the expression levels of miR-28 and miR-142 in serum; Spearman correlation was applied to analyze the correlation between the expression levels of miR-28 and miR-142 in the serum of AIS patients and the scores of the National Institutes of Health Stroke Scale (NIHSS) and the Modified RankinScale (MRS); Logistic regression was applied to analyze factors that affected the prognosis of AIS patients; ROC curve was applied to an-alyze the predictive value of serum miR-28 and miR-142 expression levels for the prognosis of AIS patients.Results The expression levels of miR-28 (0.64±0.07 vs. 1.01±0.10) and miR-142 (0.55±0.06 vs. 1.02±0.07) in the serum of patients in AIS group were lower than those in the control group (P<0.05), The levels of miR-28 (0.87±0.10, 0.69±0.09, 0.48±0.08) and miR-142 (0.63±0.07, 0.56±0.06,0.50±0.05) in the serum of AIS patients in the mild stroke group, moderate stroke group, and severe stroke group decreased significant-ly in sequence (P<0.05), The expression levels of miR-28 (0.55±0.10 vs. 0.71±0.08) and miR-142 (0.50±0.05 vs. 0.59±0.07) in the se-rum of AIS patients in poor prognosis group were lower than those in good prognosis group (P<0.05). Spearman correlation analysis re-sults showed that the expression levels of miR-28 and miR-142 were negatively correlated with NIHSS scores and MRS scores, respec-tively (P<0.05). Compared with good prognosis group, the proportion of patients with hypertension and/or diabetes, systolic blood pres-sure, diastolic blood pressure and low-density lipoprotein cholesterol in poor prognosis group were significantly higher in poor prognosis group (P<0.05). The results of logistic regression analysis showed that miR-28, miR-142, the proportion of patients with hypertension and/or diabetes and systolic blood pressure were factors affecting the poor prognosis of AIS patients (P<0.05). ROC curve analysis showed that the combination of miR-28 and miR-142 in serum in predicting the prognosis of AIS is more effective than a single indica-tor of miR-28 or miR-142.Conclusion The decreased expression levels of miR-28 and miR-142 in the serum of AIS patients are relat-ed to the severity of AIS, and the combination of the two has high predictive value for the prognosis of AIS.
