| 体外膜肺氧合( ECMO)作为一种体外生命支持技术,能部分或全面代替心脏和 /或肺功能,为重症呼吸或循环衰竭病人提供关键支持。 ECMO主要分为静脉 -静脉( VV)和静脉 -动脉( VA)两种模式。 VV-ECMO通过从静脉抽血,经过体外膜肺进行氧合后返还,辅助肺功能;而 VA-ECMO既能替代肺功能,也可支持心脏工作,通过将氧合血液送回主动脉,提高全身氧供。 ECMO技术能有效防止低氧血症、减轻肺损伤,提升主动脉血流量和器官灌注,适用范围日渐广泛。现综述 ECMO的工作原理、插管技巧及其局限,为临床提供指导。 |

| Extracorporeal membrane oxygenation (ECMO) is an advanced life support system designed to take over cardiac and/or pul.monary functions partially or completely, providing essential aid to patients with critical respiratory or cardiac conditions. ECMO isbroadly divided into two types: veno-venous (VV) and veno-arterial (VA). VV-ECMO extracts blood from a vein, re-oxygenates it using an extracorporeal membrane, and reintroduces it, thereby supporting pulmonary function. VA-ECMO, on the other hand, not only facili. tates lung function but also supports cardiac activity by returning oxygen-rich blood directly into the aorta, improving systemic oxygen.ation. The use of ECMO technology helps prevent hypoxemia, minimizes lung damage, enhances aortic blood flow and organ perfusion,and its applications in clinical settings continue to grow. The article offers a detailed examination of the operational principles, intuba.tion techniques, and limitations of ECMO, serving as a practical guide for clinical implementation. |
