| 目的探讨血清可溶性髓样细胞触发受体 2(sTREM2)胰岛素样生长因子 -1(IGF-1)与 2型糖尿病( T2DM)病人轻度认知功能障碍( MCI)的相关性及其诊断价值。方法选取 2021年,2月至 2023年 3月于咸阳市第一人民医院就诊的 126例 T2DM病人为 T2DM组,另选取同时期体检正常的志愿者 101例为健康组。收集两组的临床资料;采用酶联免疫吸附测定( ELISA)检测血清 sTREM2、IGF-1的表达水平; Pearson相关性分析血清 sTREM2、IGF-1与 MoCA评分的相关性;多因素 logistic回归分析 T2DM病人发生 MCI的影响因素;受试者操作特征曲线( ROC曲线)分析血清 sTREM2、IGF-1表达水平对 T2DM病人发生 MCI的诊断价值。结果与健康组相比, T2DM组病人血清 sTREM2表达水平上升[(252.71±61.62)ng/L比(165.32±32.87)ng/L],IGF-1表达水平下降[( 86.38±17.67)μg/L比( 123.54±22.85)μg/L](P<0.05); MCI组病人血清 sTREM2表达水平高于非 MCI组[(286.89±51.35)ng/L比( 212.64±46.91)ng/L],IGF-1水平低于非 MCI组[(76.67±11.51)μg/L比( 97.77±16.85)μg/L](P<0.05);临床资料分析,两组病人的年龄、糖尿病病程、受教育年限、 MoCA评分比较差异有统计学意义( P<0.05); Spearman相关性分析表明,血清 sTREM2水平与 MoCA评分呈正相关, IGF-1水平与 MoCA评分呈负相关( r=0.43、r=.0.51,P<0.05);多因素 logistic回归分析显示年龄、糖尿病病程、受教育年限、 MoCA评分、 sTREM2、IGF-1均是 T2DM病人发生 MCI的影响因素( P<0.05); ROC曲线分析血清 sTREM2、IGF-1及二者联合诊断 T2DM病人发生 MCI的曲线下面积( AUC)分别为 0.89、0.88、0.96,二者联合诊断相比于血清 sTREM2、IGF-1单独诊断更具优势( Z二者联合 -sTREM2=2.85、P=0.004,Z二者联合 -IGF-1=3.26、P=0.001)。结论 T2DM病人血清中 sTREM2表达水平上升, IGF-1表达水平下降,两者与 MCI的发生密切相关,两者表达水平可以作为诊断 T2DM病人发生 MCI的潜在生物学标志物。 |

| Objective To investigate the correlation between serum soluble triggering receptor expressed on myeloid cells 2 (sTREM2), insulin-like growth factor-1 (IGF-1) and mild cognitive impairment (MCI) in patients with type 2 diabetes mellitus (T2DM) and their diagnostic value.Methods A total of 126 T2DM patients, who visited Xianyang First People's Hospital from February 2021to March 2023, were regarded as the T2DM group, and 101 volunteers who underwent normal physical examinations as control group.Clinical data were gathered. Enzyme linked immunosorbent assay (ELISA) was applied to detect the expression levels of serum sTREM2 and IGF-1; Pearson correlation was performed to explore the relevance between serum sTREM2, IGF-1, and MoCA score; mul.tivariate logistic regression was carried out to discuss the factors influencing MCI in T2DM patients; receiver operating characteristiccurve (ROC curve) was applied to analyze the diagnostic value of serum sTREM2 and IGF-1 expression levels for MCI in T2DM pa. tients.Results Compared with control group, the serum sTREM2 expression level in the T2DM group increased [(252.71±61.62) ng/L vs. (165.32±32.87) ng/L], while the IGF-1 expression level decreased [(86.38±17.67) μg/L vs. (123.54±22.85) μg/L] (P<0.05). The se.rum sTREM2 expression level in the MCI group had a higher serum sTREM2 expression level than non MCI group [(286.89±51.35) ng/L vs. (212.64±46.91) ng/L], and the IGF-1 level was lower than that in the non MCI group [(76.67±11.51) μg/L vs. (97.77±16.85) μg/L] (P<0.05). Clinical data analysis results showed that there were differences between the two groups in age, course of diabetes, years ofeducation and MoCA score (P<0.05). Pearson correlation analysis revealed that serum sTREM2 was positively correlated with MoCA score, while IGF-1 was negatively correlated with MoCA score (r=0.43, r=.0.51, P<0.05). Multivariate logistic regression analysis re.vealed that age, course of diabetes, years of education, MoCA score, sTREM2, and IGF-1 were all the influencing factors for MCI in T2DM patients (P<0.05). The areas under the curve (AUC) of serum sTREM2, IGF-1, and their combined diagnosis for MCI in T2DMpatients using ROC curve analysis were 0.89, 0.88, and 0.96, respectively. The combined diagnosis of the two had more advantagescompared to individual diagnosis (Zcombined diagnosis-STEM2=2.85, P=0.004, Zcombined diagnosis-IGF-1=3.26, P=0.001). Conclusion The increased ex. pression level of sTREM2 and decreased level of IGF-1 in the serum of T2DM patients are closely associated with the occurrence ofMCI. The expression levels of both can serve as potential biological markers for diagnosing MCI in T2DM patients. |
