| 目的探究 2型糖尿病( T2DM)合并干眼病人血清可溶性细胞间黏附分子 -1(sICAM-1)、白细胞介素 -20(IL-20)水平与糖脂代谢水平及干眼严重程度的相关性。方法回顾性选择 2019年 6月至 2023年 6月在沧州爱尔眼科医院诊治的 148例 T2DM病人,根据是否合并干眼分为 T2DM组(68例)和干眼组( 80例)。酶联免疫吸附分析检测 sICAM-1和 IL-20水平; Schirmer Ⅰ试验、非接触式泪膜破裂时间(NIBUT)标准眼表疾病指数(OSDI)评价干眼严重程度;全自动生化分析仪检测总胆固醇(TC)、甘油三酯( TG)、低密度脂蛋白胆固醇( LDL-、C)、高密度脂蛋白胆固醇( HDL-C);血糖仪检测空腹血糖( FPG)及餐后 2h血糖( 2h PPG)。血红蛋白分析仪测定糖化血红蛋白( HbA1C)水平,全自动免疫分析仪测定空腹胰岛素( FINS)水平。采用 Pearson相关性分析探讨 sICAM-1、IL-20与干眼严重程度及糖脂代谢的相关性;受试者操作特征曲线( ROC曲线)分析 sICAM-1、IL-20对 T2DM病人发生干眼的诊断价值。结果干眼组 sICAM-1水平[( 458.27±131.25)μg/L比( 317.64±94.48)μg/L]、 IL-20水平[( 537.82±126.31)ng/L比( 412.24±93.84)ng/L]、 OSDI评分[( 31.63±9.76)分比( 15.32±3.75)分]、 TG水平[( 1.92±0.47)mmol/L比(1.54±0.42)mmol/L]、 TC水平[( 5.32±1.17)mmol/L比( 4.87±1.02)mmol/L]、 LDL-C水平[( 3.84±0.87)mmol/L比( 3.12±0.62)mmol/L]、 2 h PPG水平[( 11.67±2.56)mmol/L比( 10.28±2.37)mmol/L]、 HbA1c水平[( 8.94±2.73)%比( 7.32±2.11)%]及 FINS水平[( 11.41±3.38)mIU/L比( 10.29±3.14)mIU/L]高于 T2DM组( P<0.05); HDL-C水平、 SchirmerⅠ值与 NIBUT值较低( P<0.05)。干眼组 sICAM-1、IL-20水平与 SchirmerⅠ值、 NIBUT值负相关( P<0.05)与 OSDI评分及 TG、TC、LDL-C、2 h PPG、HbA1c正相关( P <0.05)。 sICAM-1、IL-20诊断 T2DM病人发生干眼的曲线下面积( AUC)分别为 0.83、0.85,两者联合诊断 AUC为 0.94,优于单独诊断( P<0.05)。结论 T2DM合并干眼病人血清 sICAM-1、IL-20与糖脂代谢及干眼严重程度正相关,联合诊断 T2DM病人发生干眼具有较大的诊断价值。 |

| Objective To explore the correlation between the expression of soluble intercellular adhesion molecule-1 (sICAM-1) and interleukin-20 (IL-20) in serum and the level of glucose and lipid metabolism and the severity of dry eye in type 2 diabetes mellitus(T2DM) patients with dry eye disease (DED).Methods Retrospective selection of 148 patients with T2DM who were diagnosed andtreated in Cangzhou Aier Ophthalmology Hospital from June 2019 to June 2023 were assigned to T2DM group (n=68) and DED group (n =80) according to whether they were complicated with DED. The levels of sICAM-1 and IL-20 were detected by enzyme-linked immuno-sorbent assay (ELISA). The severity of dry eye was evaluated by SchirmerⅠ test, non-invasive break-up time (NIBUT) and standard oc-ular surface disease index (OSDI). Total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and high densi-ty lipoprotein cholesterol (HDL-C) were detected by automatic biochemical analyzer. Fasting plasma glucose (FPG) and 2-hour post-prandial plasma glucose (2-hour PPG) were detected by blood glucose meter. Glycosylated hemoglobin (HbA1C) was measured by he-moglobin analyzer, and fasting insulin (FINS) was measured by fully automated immunoassay analyzer. Pearson correlation analysis wasmade to analyze the correlation between sICAM-1, IL-20 and DED severity glucose and lipid metabolism. The diagnostic value of sI-CAM-1 and IL-20 for the development of DED in T2DM patients was analyzed using receiver operating characteristic curve (ROC curve).Results The levels of sICAM-1 [(458.27±131.25) μg/L vs. (317.64±94.48) μg/L], IL-20 [(537.82±126.31) ng/L vs. (412.24±93.84) ng/L], OSDI score [(31.63±9.76) points vs. (15.32±3.75) points], TG [(1.92±0.47) mmol/L vs. (1.54±0.42) mmol/L], TC [(5.32± 1.17) mmol/L vs. (4.87±1.02) mmol/L], LDL-C [(3.84±0.87) mmol/L vs. (3.12±0.62) mmol/L], 2-hour PPG [(11.67±2.56) mmol/L vs. (10.28±2.37) mmol/L], HbA1c [(8.94±2.73) % vs. (7.32±2.11) %], and FINS [(11.41±3.38) mIU/L vs. (10.29±3.14) mIU/L] in the dry eye group were higher than those in the T2DM group (P<0.05). HDL-C level, SchirmerⅠ value and NIBUT value were low in DED group (P<0.05). The levels of sICAM-1 and IL-20 in DED group were negatively correlated with SchirmerⅠ value and NIBUT value (P <0.05), while positively correlated with OSDI score, TG, TC, LDL-C, 2-hour PPG and HbA1c (P<0.05). The AUCs of sICAM-1 and IL-20 for diagnosing DED in T2DM patients were 0.83 and 0.85, respectively. The AUC of the combined diagnosis of SICAM-1 and IL-20 was 0.94, which was superior to that of either alone (P<0.05).Conclusions Serum sICAM-1 and IL-20 in T2DM patients with DEDare positively correlated with glucose and lipid metabolism and the severity of dry eye. Combined diagnosis of DED in T2DM patientshas great diagnostic value and can be used for clinical analysis. |
