| 目的探究血清非转移性黑色素瘤糖蛋白 B(glycoprotein non-metastatic melanoma protein B,GPNMB)、自噬相关蛋白 1(Beclin-1)在白内障病人术后眼内炎诊断中的应用价值。方法前瞻性研究收集 2021年 8月至 2023年 8月在张家口市第四医院进行手术治疗的 131例白内障病人作为观察对象(白内障组)根据是否发生眼内炎感染,分为非眼内炎组 90例和眼内炎组 41例,并选取同期在该院体检的健康者 118例作为健康组,采用酶联免,疫吸附分析测定血清中 GPNMB、Beclin-1水平, logistic回归分析探索白内障病人术后眼内炎发生的影响因素,受试者操作特征曲线(ROC曲线)分析血清 GPNMB、Beclin-1水平对白内障病人术后眼内炎发生的诊断价值。结果与健康组比较,白内障组血清 GPNMB水平[(4.59±1.36)μg/L比(1.58±0.39)μg/L]显著升高, Beclin-1水平[(0.77±0.19)μg/L比(1.34±0.23)μg/L]显著降低( P<0.05)。非眼内炎组和眼内炎组在性别、年龄、饮酒史、吸烟史、高血压、糖尿病比较差异无统计学意义( P>0.05);眼内炎组手术时间 ≥30 min、有玻璃体溢出的病人比例及 GPNMB水平[(5.18±1.02)μg/L比( 4.32±0.62)μg/L]显著高于非眼内炎组, Beclin-1水平[(0.70±0.09)μg/L比( 0.80±0.11)μg/L]显著低于非眼内炎组( P<0.05)。玻璃体溢出、手术方法为普通白内障摘除法、 GPNMB是白内障病人术后眼内炎发生的独立危险因素, Be-clin-1是保护因素( P<0.05)。 GPNMB、Beclin-1水平和二者联合诊断白内障病人术后发生眼内炎的曲线下面积分别为 0.78、0.78、0.86,二者联合诊断价值优于单独诊断( Z=2.42、2.02;P=0.015、0.044)。结论白内障病人血清中 GPNMB水平升高, Be-clin-1水平降低,对白内障术后眼内炎的诊断具有重要的价值,可以作为白内障术后眼内炎诊断的辅助血清标志物。 |

| Objective To explore the application value of serum glycoprotein non-metastatic melanoma protein B (GPNMB) and Be-clin-1 in the diagnosis of postoperative endophthalmitis in cataract patients.Methods Prospective study,a total of 131 cataract pa-tients who underwent surgical treatment in the Fourth Hospital of Zhangjiakou from August 2021 to August 2023 were collected as ob-servation subjects (cataract group), according to whether endophthalmitis infection occurred, there were 90 cases in the non endophthal-mitis group and 41 cases in the endophthalmitis group, and 118 healthy individuals who underwent physical examination in the hospitalwere collected as the health group; enzyme-linked immunosorbent assay (ELISA) was applied to analyze the levels of GPNMB and Be-clin-1 in serum; Logistic regression was applied to analyze the influencing factors of postoperative endophthalmitis in cataract patients;receiver operating characteristic curve (ROC curve) was applied to analyze the diagnostic value of serum GPNMB and Beclin-1 levels for postoperative endophthalmitis in cataract patients.Results Compared with the health group, the serum GPNMB level in the cata-ract group [(4.59±1.36) μg/L vs. (1.58±0.39) μg/L] was obviously increased, while the Beclin-1 level [(0.77±0.19) μg/L vs. (1.34±0.23) μg/L] was obviously reduced (P<0.05). There was no obvious difference between non endophthalmitis group and endophthalmitis groupin gender, age, drinking history, smoking history, hypertension, diabetes (P>0.05); the proportions of patients with surgery time ≥ 30minutes, vitreous overflow, and the level of GPNMB in the endophthalmitis group [(5.18±1.02) μg/L vs. (4.32±0.62) μg/L] were obvious-ly higher than those in the non endophthalmitis group, while the level of Beclin-1 [(0.70±0.09) μg/L vs. (0.80±0.11) μg/L] was obvious-ly lower than that in the non endophthalmitis group (P<0.05). Vitreous overflow, the surgical methods were common cataract extractionand GPNMB were independent risk factors for postoperative endophthalmitis in cataract patients, while Beclin-1 was a protective factor (P<0.05). The AUC of postoperative endophthalmitis in cataract patients diagnosed with GPNMB, Beclin-1 levels, and their combination was 0.78, 0.78, and 0.86, respectively, the combined diagnostic value of the two was superior to that of individual diagnosis (Z=2.42, 2.02; P=0.015, 0.044).Conclusion The level of GPNMB in the serum of cataract patients increases, while the level of Beclin-1 de-creases, they have important value in the diagnosis of postoperative endophthalmitis and can serve as auxiliary serum markers for the di-agnosis of postoperative endophthalmitis. |
