Objective: To describe changes in the co-occurrence of depressive symptoms and somatic multimorbidity among middle-aged and older Chinese adults and to examine its association with incident cardiovascular disease. Methods: Data were drawn from the 2011, 2013, 2015, 2018, and 2020 waves of the China Health and Retirement Longitudinal Study. The analysis included 10,101 adults aged 45 years or older who had no heart disease or stroke in 2011 and had complete data on the main study variables. Depressive symptoms were assessed with the 10-item Center for Epidemiologic Studies Depression Scale. Somatic multimorbidity was defined as the presence of at least two of nine non-cardiovascular chronic conditions. At each wave, participants were classified as having neither condition, depressive symptoms only, somatic multimorbidity only, or both conditions. Each wave-specific health state was related to incident heart disease or stroke at the following wave using discrete-time complementary log-log models. Results: The analysis included 30,657 person-periods and 2,112 composite cardiovascular events, including 1,584 cases of heart disease and 605 cases of stroke. Between 2011 and 2018, the prevalence of co-occurring depressive symptoms and somatic multimorbidity increased from 13.0% to 22.1%, while the prevalence of somatic multimorbidity alone increased from 14.0% to 25.9%. The proportion with neither condition decreased from 51.3% to 37.0%. After full adjustment, the hazard ratios for incident composite cardiovascular disease were 1.38 (95% CI: 1.20–1.58) for depressive symptoms only, 1.97 (95% CI: 1.76–2.21) for somatic multimorbidity only, and 2.51 (95% CI: 2.23–2.82) for both conditions, compared with neither condition (all P<0.001). For participants with both conditions, the hazard ratios were 2.45 (95% CI: 2.14–2.81) for heart disease and 2.74 (95% CI: 2.22–3.39) for stroke. No statistically significant multiplicative interaction was observed. The association remained positive in analyses using spline adjustment, accounting for death as a competing event category, and multiple imputation. Conclusion: The co-occurrence of depressive symptoms and somatic multimorbidity became more common over time and was associated with the highest risk of subsequent cardiovascular disease.
