      目的分析血清外泌体微 RNA(miR)-21-5p、Ras同源基因家族成员 B(RhoB)水平变化与胃癌病人临床病理特征、化疗疗效的相关性。方法前瞻性选取 2020年 8月至 2024年 8月在开滦总医院进行治疗的胃癌晚期病人 106例作为观察组,根据化疗效果分为化疗敏感组( n=41)和化疗耐药组( n=65),另选同期进行体检的健康志愿者 106例为健康组,经实时荧光定量逆转录聚合酶链反应( qRT-PCR)检测血清外泌体 miR-21-5p相对表达量,酶联免疫吸附分析( ELISA)检测血清 RhoB水平,收集分析一般临床资料,相对危险度分析 miR-21-5p、RhoB表达对病人化疗效果的影响,绘制受试者操作特征曲线( ROC曲线)分析 miR-21-5p、RhoB对病人化疗效果的预测价值, Pearson法分析 miR-21-5p、RhoB之间相关性。结果观察组的血清 miR-21-5p水平 1.56±0.17比 1.01±0.11较健康组显著升高( P<0.05)RhoB水平[(35.76±3.94)ng/L比( 46.95±4.85)ng/L]较健康组显著降低(P<0.05);不同 TNM分期、分化程度、远处转移以及病灶个,数的胃癌病人 miR-21-5p、RhoB水平差异有统计学意义(P<0.05);疗耐药组的血清 miR-21-5p表达量 1.68±0.19比 1.36±0.15较化疗敏感组显著升高( P<0.05),RhoB水平[( 32.65±3.58)ng/L比化(40.68±4.51)ng/L]较化疗敏感组显著降低( P<0.05)两组病人的 TNM分期、分化程度之间差异有统计学意义( P<0.05); miR-21-5p与 RhoB呈负相关( r=.0.45,P<0.05); miR-21-5p与,RhoB联合对病人化疗效果的预测 AUC显著高于 miR-21-5p(Z=2.40,P =0.016)、 RhoB(Z=2.24,P=0.025)单独预测; miR-21-5p高表达病人的化疗耐药风险高于 miR-21-5p低表达病人, RhoB低表达病人的化疗耐药风险高于 RhoB高表达病人( P<0.05)。结论胃癌病人血清外泌体 miR-21-5p水平升高, RhoB水平降低,且与病

      Objective To analyze the correlation between changes in levels of serum exosomal microribonucleic acid (miR-21-5p)and Ras homolog gene family member B (RhoB) with clinical pathological features and chemotherapy efficacy in gastric cancer patients.Methods From August 2020 to August 2024, prospective selection of 106 gastric cancer patients who received treatment in KailuanGeneral Hospital were included as the observation group. According to the chemotherapy effect, they were assigned into chemotherapysensitive group (n=41) and chemotherapy resistant group (n=65). Another 106 healthy volunteers who underwent physical check ups were as the health group. Real-time fluorescent quantitative reverse transcription-polymerase chain reaction (qRT-PCR) was used to de-tect the relative expression level of serum extracellular vesicle miR-21-5p. Enzyme-linked immunosorbent assay (ELISA) was used todetect serum RhoB level. General clinical data were collected and analyzed. Relative risk analysis was made of the impact of miR-21-5p, RhoB expression on patients' chemotherapy outcome. The receiver operating characteristic curve (ROC curve) was plotted to ana-lyze the predictive value of miR-21-5p and RhoB on the efficacy of chemotherapy in patients. Pearson method was used to analyze the correlation between miR-21-5p and RhoB. Results The serum miR-21-5p (1.56±0.17 vs. 1.01±0.11) in the observation group was greatly higher than that in the health group (P<0.05), and the RhoB [(35.76±3.94) ng/L vs. (46.95±4.85) ng/L] was greatly lower than that in the health group (P<0.05). There was a statistically great difference in miR-21-5p and RhoB among gastric cancer patients withdifferent TNM staging, differentiation degree, distant metastasis, and number of lesions (P<0.05). The expression level of serum miR-21-5p (1.68±0.19 vs. 1.36±0.15) in the chemotherapy resistant group was higher than that in the chemotherapy sensitive group (P< 0.05), and the level of RhoB [(32.65±3.58) ng/L vs. (40.68±4.51) ng/L] was lower than that in the chemotherapy sensitive group (P< 0.05). The difference in TNM staging and differentiation degree between the two groups was statistically significant (P<0.05). MiR-21-5p was negatively correlated with RhoB (r=.0.45, P<0.05). The AUC of the combination of miR-21-5p and RhoB in predicting chemo-therapy efficacy was greatly higher than that of miR-21-5p (Z=2.40, P=0.016) and RhoB (Z=2.24, P=0.025) alone. The risk of chemo-therapy resistance was higher in patients with high miR-21-5p expression than in patients with low miR-21-5p expression, and the riskof chemotherapy resistance was higher in patients with low RhoB expression than in patients with high RhoB expression (P<0.05).Con. clusion The level of serum extracellular vesicle miR-21-5p is elevated and the level of RhoB is decreased in gastric cancer patients,which has a certain correlation with pathological features and chemotherapy efficacy.
