| 目的探究特应性皮炎( AD)病儿皮肤菌群特征并分析其与皮肤屏障功能的关系。方法前瞻性选取 2022年 1月至 2024年 1月于郑州大学附属郑州中心医院就诊的 104例 AD病儿作为研究组,另选取 104例体检健康儿童作为对照组,取研究组面部、躯干、前臂等各个皮损位置皮肤样本,对照组同样采集相应位置皮肤样本。通过 Illumina Miseq2高通量测序仪对皮肤表面菌群 16S核糖体核糖核酸基因 V3~V4可变区进行测序,分析其皮肤菌群的组成结构并比较;根据研究组病儿皮肤屏障功能障碍情况将其分为皮肤屏障功能障碍组和皮肤屏障功能非障碍组,比较两组病儿的皮肤菌群特征;以经皮水分丢失(TEWL)值评价 AD病儿皮肤屏障功能,采用 Pearson相关性分析 AD病儿皮肤菌群特征与 TEWL值的关系。结果研究组病儿皮肤菌群操作分类单元( OTUs)数低于对照组;研究组病儿皮肤菌群基于丰度的覆盖估计值( ACE)指数( 221.42±30.78比288.59±36.06)、 Chao1指数( 183.74±28.66比 291.55±42.08)和 Shannon指数( 2.74±0.45比 3.88±0.90)均低于对照组( P<0.05);与对照组相比,研究组病儿皮肤菌群在属水平上葡萄球菌相对丰度升高,双歧杆菌、乳杆菌、丙酸杆菌相对丰度降低,在种水平上金黄色葡萄球菌相对丰度升高,表皮葡萄球菌、人葡萄球菌、乳酸杆菌、费氏丙酸杆菌相对丰度降低( P<0.05); 104例 AD病儿皮肤屏障功能障碍发生率为 23.08%(24/104),与皮肤屏障功能非障碍组相比,皮肤屏障功能障碍组病儿皮肤菌群在属水平上葡萄球菌相对丰度升高,双歧杆菌、乳杆菌、丙酸杆菌相对丰度降低,在种水平上金黄色葡萄球菌相对丰度升高,表皮葡萄球菌、人葡萄球菌、乳酸杆菌、费氏丙酸杆菌相对丰度降低( P<0.05); Pearson相关分析结果显示 AD病儿皮肤表面的葡萄球菌属、金黄色葡萄球菌与 TEWL值呈正相关( P<0.05)双歧杆菌属、乳杆菌属、丙酸杆菌属、表皮葡萄球菌、人葡萄球菌、乳酸杆菌、费氏丙酸杆菌与 TEWL值呈负相关( P<0.05)。结AD病儿皮肤菌群丰富度及多样性降低,皮肤菌群特征与 AD病儿皮肤屏障论,功能障碍关系密切。 |

| Objective To explore the characteristics of skin flora in children with atopic dermatitis (AD) and to analyze their relation-ship with skin barrier function.Methods Prospective selection of 104 children with AD who visited Zhengzhou University AffiliatedZhengzhou Central Hospital from January 2022 to January 2024 were selected as the study group, and 104 healthy children undergoingphysical examinations were selected as the control group. Skin samples from various lesion locations, such as the face, body, and fore-arms, were collected from the study group, and the corresponding skin samples were also collected from the control group. The V3~V4variable regions of the 16S ribosomal ribonucleic acid gene of the skin surface microbiota were sequenced using Illumina Miseq2 high-throughput sequencer, and the compositional structures of the skin microbiota were analyzed and compared. Children in the studygroup were assigned to a group with skin barrier dysfunction or a group without skin barrier dysfunction according to the status of theirskin barrier, and the skin microbiota characteristics of the two groups were compared. The skin barrier function of children with AD wasevaluated by transepidermal water loss (TEWL) value, and the relationship between skin microbiota characteristics and TEWL valuewas analyzed by Pearson correlation.Results The number of operational taxonomic units (OTUs) of the skin microbiota in the studygroup was lower than that in the control group. The Abundance-based Coverage Estimator (ACE) index (221.42 ± 30.78 vs. 288.59 ± 36.06), Chao1 index (183.74 ± 28.66 vs. 291.55 ± 42.08), and Shannon index (2.74 ± 0.45 vs. 3.88 ± 0.90) of the skin microbiota in the study group were lower than those in the control group (P<0.05). Compared with the control group, the relative abundance of Staphylo. coccus in the skin microbiota of children in the study group increased at the genus level, while the relative abundance of Bifidobacteri. um, Lactobacillus, and Propionibacterium decreased. At the species level, the relative abundance of Staphylococcus aureus increased, while the relative abundance of Staphylococcus epidermidis, Staphylococcus hominis, Lactobacillus, and Propionibacterium fischeri de-creased (P<0.05). The incidence of skin barrier dysfunction in 104 children with AD was 23.08% (24/104). Compared with the groupwithout skin barrier dysfunction, the relative abundance of Staphylococcus in the group with skin barrier dysfunction increased at the ge-nus level, while the relative abundance of Bifidobacterium, Lactobacillus, and Propionibacterium decreased at the genus level. At the species level, the relative abundance of Staphylococcus aureus increased, while the relative abundance of Staphylococcus epidermidis, Staphylococcus hominis, Lactobacillus, and Propionibacterium fischeri decreased, and the differences were statistically significant (P< 0.05). The Pearson correlation analysis results showed that Staphylococcus and Staphylococcus aureus on skin surface of AD children were positively correlated with TEWL value (P<0.05), while Bifidobacterium, Lactobacillus, Propionibacterium, Staphylococcus epidermi. dis, Staphylococcus hominis, Lactobacillus and Propionibacterium fischeri were negatively correlated with TEWL value (P<0.05).Conclu. sion In children with AD, the richness and diversity of skin microbiota are decreased, and the characteristics of skin microbiota areclosely associated with skin barrier dysfunction. |
