| 目的分析超声定性特征与乳腺癌病理类型、分化及浸润转移的相关性。方法前瞻性选取 2019年 2月至 2024年 1月芜湖市中医医院收治的 82例乳腺癌病人为研究对象,均行超声检查观察超声定性特征,探究超声定性特征与病理类型、分化及浸润转移的关系及其对淋巴结转移的影响。结果浸润性导管癌形态不规则、钙化、淋巴结转移占比高于浸润性小叶癌( P <0.05);低分化乳腺癌形态不规则、钙化、血流信号 Ⅱ~Ⅲ级、淋巴结转移占比高于中高分化( P<0.05);淋巴结转移乳腺癌的病灶最大径、距乳头距离 0~2 cm比例、边缘毛刺征比例、血流信号 Ⅱ~Ⅲ级比例分别为( 2.25±0.33)cm、18.60%(8/43)、 74.42%(32/ 43)、 79.07%(34/43)大于未淋巴结转移乳腺癌的[(2.10±0.31)cm、7.69%(3/39)、 51.28%(20/39)、 53.85%(21/39)](P<0.05);多因素 logistic回归分析结,果显示病灶最大径、距乳头距离、边缘毛刺征、血流信号均为乳腺癌淋巴结转移的独立影响因素( P<0.05)。结论超声定性特征形态、血流信号、钙化、淋巴结转移与乳腺癌病理类型、分化程度及淋巴结转移联系密切。 |

| Objective To analyze the correlation between qualitative ultrasound characteristics and breast cancer pathological type,differentiation, infitration and metastasis.Methods Prospective selection of 82 breast cancer patients admitted to Wuhu Hospital ofTraditional Chinese Medicine from February 2019 to January 2024 were selected as research subjects. All patients underwent ultra-sound examination to observe the qualitative characteristics. The relationship of these characteristics with pathological type, differentia-tion, infitration and metastasis, as well as their impact on lymph node metastasis, was explored.Results The proportions of irregularmorphology, calcification, and lymph node metastasis in invasive ductal carcinoma were higher than those in invasive lobular carcino-ma (P<0.05). Low-differentiated breast cancer had higher proportions of irregular morphology, calcification, blood flow signal grade Ⅱ-Ⅲ, and lymph node metastasis than medium-and high-differentiated breast cancer (P<0.05). In breast cancer with lymph node metasta-sis, the maximum lesion diameter, and the proportions of lesions located 0-2 cm from the nipple, spiculated margin, and blood flow sig-nal grade Ⅱ-Ⅲ were (2.25±0.33) cm, 18.60% (8/43), 74.42% (32/43), and 79.07% (34/43), respectively, which were higher than thosein breast cancer without lymph node metastasis [(2.10±0.31) cm, 7.69% (3/39), 51.28% (20/39), and 53.85% (21/39)] (P<0.05). The re-sults of the multivariate logistic regression analysis showed that the maximum diameter of the lesion, distance from the nipple, spiculat-ed margin, and blood flow signal were all independent factors affecting lymph node metastasis in breast cancer (P<0.05).Conclusion The qualitative characteristics of ultrasound, such as morphology, blood flow signal, calcification, and lymph node metastasis, are close-ly associated with the pathological type, differentiation grade, and lymph node metastasis of breast cancer. |
