      目的探究极早产与超早产儿过快追赶生长时神经发育状况及早期干预。方法前瞻性选取秦皇岛市妇幼保健院 2019年 1月至 2024年 1月就诊的极早 /超早产儿 156例为研究对象。根据产妇生产时的孕周将总体分为两组:极早产组( 28~32周) 86例和超早产组( 24~28周) 70例,然后组内进一步按基线特征细分亚层,使用统计学软件 SPSS 23.0生成随机序列,按亚层比例分配样本,对性别、胎龄、出生体质量等基线特征进行协变量匹配,确保组间基线特征均衡,最终从极早产组 86例中选择 65例确定为极早产组,而从 70例超早产组中选择 65例确定为超早产组。观察并评估两组的体格发育情况, Gesell发育量表(GDS)评分,智力发育指数( MDI)和神经发育损害( NDI)情况。结果干预后极早产儿组整体体格发育情况优于超早产儿组(P<0.05);经干预后两组早产儿 GDS评分较前均有提升,但极早产儿的 GDS评分整体优于超早产儿(P<0.05);干预后极早产儿组矫正 6月龄时 MDI评分(80.21±10.52)分及 12月龄 MDI评分(90.02±10.42)分均显著优于超早产儿组 6月龄( 76.17±9.09)分及 12月龄( 86.36±9.39)分(均 P<0.05);而极早产儿组矫正 6月龄时 PDI评分( 76.18±11.07)分及 12月龄 PDI评分( 87.86±12.05)分也显著优于超早产儿组 6月龄( 72.02±9.53)分及 12月龄( 83.43±11.47)分(均 P<0.05);极早产儿组 NDI概率低于超早产儿组(1.54%比 10.77%),但两组间差异无统计学意义( P>0.05)。结论过快追赶生长一定程度上有利于极早产与超早产儿神经发育,改善预后。

      Objective To explore the current status of neural development and early intervention during rapid catch-up growth in ex-tremely premature and ultra premature infants. Methods Prospective selection of 156 extremely preterm/very preterm infants whowere treated at the Qinhuangdao Maternal and Child Health Hospital from January 2019 to January 2024 were selected as the researchsubjects. The overall population was divided into two groups according to the gestational age at delivery: 86 cases in the extremely pre.term group (28-32 weeks) and 70 cases in the very preterm group (24-28 weeks). Then, within each group, subgroups were further divid-ed according to baseline characteristics. The statistical software SPSS 23.0 was used to generate random sequences, and samples wereallocated according to the proportion of each subgroup. Covariate matching was performed for baseline characteristics such as gender,gestational age and birth weight to ensure the balance of baseline characteristics between groups. Finally, 65 cases were selected from86 cases in the extremely preterm group as the extremely preterm group, and 65 cases were selected from 70 cases in the very pretermgroup as the very preterm group. The physical development, Gesell developmental scale (GDS) score, mental development index (MDI),and neurodevelopmental impairment (NDI) of the two groups were observed and evaluated. Results After intervention, the overallphysical development of the extremely preterm infant group was better than that of the very preterm infant group (P<0.05). The GDSscores of both groups of preterm infants improved after intervention, but the overall GDS scores of extremely preterm infants were betterthan those of very preterm infants (P<0.05). After intervention, the MDI scores of the extremely preterm infant group at corrected 6months (80.21±10.52) points and 12 months (90.02±10.42) points were significantly higher than those of the extremely preterm infant group at corrected 6 months (76.17±9.09) points and 12 months (86.36±9.39) points, with all P<0.05. Meanwhile, the PDI scores in theextremely preterm infant group at corrected 6 months (76.18±11.07) points and 12 months (87.86±12.05) points were also significantlysuperior to those in the extremely preterm infant group at the corresponding time points [(72.02±9.53) points and (83.43±11.47) points,respectively], all P<0.05. The probability of NDI in the extremely preterm infant group was lower than that in the very preterm infant group (1.54% vs. 10.77%), though there was no significant difference between the two groups (P>0.05).Conclusion Excessive catch-up growth is beneficial to the neurodevelopment and prognosis of extremely preterm and very preterm infants to a certain extent.
