| 目的报告 1例免疫功能正常的感染蜡样芽孢杆菌引起的肺脓肿病例并文献复习，增进临床医师对该病的认识。方法分析佛山市中医院 2024年 11月收治的 1例蜡样芽孢杆菌肺部感染病人，包括其临床特征及诊疗过程。结果病人主诉发热、咳嗽伴右胸痛 1周。入院诊断肺部感染，先后静脉注射哌拉西林他唑巴坦钠、亚胺培南西司他丁钠、美罗培南联合莫西沙星，影像检查提示病灶快速进展并肺脓肿形成，结合肺泡灌洗液病原靶向高通量测序结果后确诊为蜡样芽孢杆菌感染。经静脉注射万古霉素、胸腔穿刺引流脓液后病人病情好转。结论万古霉素静脉滴注及脓液引流是免疫功能正常宿主感染蜡样芽孢杆菌致肺脓肿形成的有效治疗方法。 |

| Objective To report a case of lung abscess caused by Bacillus cereus infection in an immunocompetent patient and to re.view the literature, with the aim of enhancing clinicians' understanding of this disease.Methods The clinical data of a patient withpulmonary Bacillus cereus infection admitted to Foshan Hospital of Traditional Chinese Medicine in November 2024 were analyzed, in.cluding clinical features and the diagnostic and therapeutic process.Results The patient complained of fever, cough, and right chestpain for one week. The admission diagnosis was pulmonary infection. Early treatment included sequential intravenous administration ofpiperacillin-tazobactam, imipenem-cilastatin, and meropenem combined with moxifloxacin. Imaging revealed rapid progression of thelesions and formation of a lung abscess. After combining with the results of pathogen-targeted high-throughput sequencing of bronchoal.veolar lavage fluid, the diagnosis of Bacillus cereus infection was confirmed. The patient improved after intravenous vancomycin andpercutaneous thoracic drainage of pus.Conclusion Intravenous vancomycin combined with pus drainage is an effective treatment forlung abscess caused by Bacillus cereus in immunocompetent hosts. |
