| Objective To explore the relationship between serum endotoxin, microRNA-24 (miR-24) levels and pregnancy outcomesin patients with polycystic ovary syndrome (PCOS), and to evaluate the combined predictive value of these two markers for such out-comes, so as to provide a reference for improving clinical prognosis. Methods Retrospective study. A total of 192 PCOS patients, whoconceived after ovulation induction therapy and were admitted to Tangshan Maternal and Child Health Hospital from February 2021 toAugust 2023, were selected as the PCOS group, and 100 healthy pregnant women during the same period were selected as the controlgroup. Serum endotoxin and miR-24 levels were compared between the two groups. The pregnancy outcomes of the PCOS group werestatistically analyzed, and serum endotoxin, miR-24, sex hormones [luteinizing hormone (LH), testosterone (T)], and metabolic indica-tors [fasting plasma glucose (FPG), insulin (INS), and homeostatic model assessment of insulin resistance (HOMA-IR)] were compared between PCOS patients with different pregnancy outcomes. The correlations of serum endotoxin and miR-24 with sex hormones and metabolic indicators were analyzed, the receiver operating characteristic curves (ROC curves) of the subjects were plotted, and the areaunder the curve (AUC) was obtained to evaluate the predictive value of serum endotoxin and miR-24 for pregnancy outcomes. The rela-tionship between serum endotoxin, miR-24, and pregnancy outcomes in PCOS patients was also analyzed. Results The serum endotox-in level in the PCOS group was significantly higher than that in the control group, while the miR-24 level was significantly lower (P< 0.05). Among the 192 PCOS patients, 43 experienced adverse pregnancy outcomes. Compared with the subgroup with favorable preg-nancy outcomes, the subgroup with adverse pregnancy outcomes had higher serum levels of LH, T, INS, FPG, and HOMA-IR (P< 0.05).The serum endotoxin level in the adverse pregnancy outcome subgroup was higher than that in the favorable pregnancy outcome sub-group [(0.45±0.08) ng/L vs. (0.36±0.07) ng/L], and the miR-24 level was lower (0.04±0.01 vs. 0.08±0.02) (P<0.05). Pearson correlation analysis showed that serum endotoxin was positively correlated with LH, T, INS, FPG, and HOMA-IR (r = 0.68, 0.71, 0.64, 0.72, 0.72, respectively), while miR-24 was negatively correlated with LH, T, INS, FPG, and HOMA-IR (r=.0.76, .0.73, .0.62, .0.67, .0.71, re-spectively) (P< 0.05). ROC curve analysis showed that the combination of serum endotoxin and miR-24 yielded the largest AUC for pre-dicting adverse pregnancy outcomes in patients with PCOS, with a value of 0.88 [95%CI: (0.82, 0.92)], a sensitivity of 79.07%, and a specificity of 87.25% (P<0.05). Patients with PCOS and high serum endotoxin levels had a higher risk of adverse pregnancy outcomes, while those with high serum miR-24 levels had a lower risk; when both markers were present simultaneously, the interaction OR was |
