logistic回归分析显示,居住地为农村[ OR=2.41,95%CI:(1.26,4.59)]、有不良妊娠史[ OR=2.66,95%CI:(1.43,4.92)]、有动物接触史[ OR=3.18,95%CI:(1.77,5.70)]、有妊娠高血压综合征[ OR=2.25,95%CI:(1.18,4.27)]、有妊娠期糖尿病[ OR=2.75,95%CI:(1.50,5.04)]为孕早期孕妇 TORCH感染的危险因素。 |

| Objective To investigate the infection status of TORCH [Toxoplasma gondii (TOX), rubella virus (RV), cytomegalovirus(CMV), and herpes simplex virus (HSV) types Ⅰ/Ⅱ] in early pregnant women at two grade-A tertiary hospitals of Xi'an region, and to analyze the relevant influencing factors. Methods Retrospective study. A total of 2,064 pregnant women in the first trimester (gesta-tional age <14 weeks) who underwent prenatal examination at Northwest Women and Children's Hospital and the Second AffiliatedHospital of Xi'an Medical University from January 2022 to May 2024 were enrolled. TORCH-specific immunoglobulin M (IgM) antibod-ies were detected by electrochemiluminescence immunoassay. Based on the test results, the women were divided into the TORCH-posi-tive group and the TORCH-negative group. A total of 11 data points were compared between the two groups, and the indicators with sta-tistically significant differences were incorporated into a logistic regression model to screen for factors associated with TORCH infectionin early pregnancy. Results There were 158 cases of TORCH infection, including 21 cases of TOX infection, 29 cases of RV infection,47 cases of CMV infection, and 61 cases of HSV infection, yielding a total infection rate of 7.66%. There were statistically significantdifferences between the TORCH-positive group and the TORCH-negative group in terms of place of residence (urban areas: 26.58% vs. 36.78%; rural areas: 73.42% vs. 63.22%), history of adverse pregnancy outcomes (35.44% vs. 25.18%), history of animal contact (36.08% vs. 24.76%), pregnancy-induced hypertension (7.59% vs. 3.93%), gestational diabetes mellitus (9.49% vs. 4.62%), and testing season (P<0.05). Logistic regression analysis results showed that residing in rural areas [OR=2.41, 95%CI: (1.26, 4.59)], history of ad-verse pregnancy outcomes [OR=2.66, 95%CI: (1.43, 4.92)], history of animal contact [OR=3.18, 95%CI: (1.77, 5.70)], pregnancy-in-duced hypertension [OR=2.25, 95%CI: (1.18, 4.27)] and gestational diabetes mellitus [OR=2.75, 95%CI: (1.50, 5.04)] were risk factors for TORCH infection in early pregnancy. Conclusions Pregnant women in early pregnancy at two grade-A tertiary hospitals in Xi'anregion have a higher risk of TORCH infection, with CMV and HSV being the predominant pathogens. The primary risk factors are resid-ing in rural areas, history of adverse pregnancy outcomes, history of animal contact, pregnancy-induced hypertension and gestational di-abetes mellitus. Therefore, health education on TORCH infection should be provided to pregnant women with the above-mentioned characteristics, and early screening and timely intervention should be emphasized to avoid or reduce the impact of TORCH infection onpregnancy outcomes. |
