| Objective To explore what surgical approach (preservation/resection) should be taken for different pathological states ofthe twisted ovary (including torsion without necrosis, suspected torsion necrosis, and torsion necrosis) in pre-pubertal SD rats after ovar-ian torsion to minimize the damage to the reproductive function of the healthy contralateral ovary during sexual maturity. Methods The ovarian torsion animal model was established using a technique analogous to that described by Turner from October 2022 to Janu-ary 2024. Fifty-six female SD rats at the pre -sexual maturity stage with an age of 3 weeks were divided into 7 groups of 8 rats each us-ing a completely random grouping method. And one group was randomly selected to perform the simulated surgery as a control group(CG). The remaining 6 groups were respectively prepared as follows: the non -necrotic ovarian torsion reduction group (NNOR)/the non -necrotic ovarian torsion resection group (NNOO), the suspected necrotic ovarian torsion reduction group (SNOR)/the suspected necrot-ic ovarian torsion resection group (SNOO), the necrotic ovarian torsion reduction group (NOR)/the necrotic ovarian torsion resectiongroup (NOO).. Then continue to raise them until they reach 8 weeks of age, which was the period of sexual maturity. The animals weresacrificed by cervical dislocation method, and the healthy ovary was cut and paraffin-embedded for HE staining to observe the histologi-cal changes. The heart blood was retained and the serum was collected by centrifugation and the levels of related hormones that Folliclestimulating hormone (FSH), Luteinizing hormone (LH), Anti-Müllerian hormone (AMH) were detected by ELISA.Results Changes inthe structure of the healthy ovary during the period of sexual maturity: In the CG group, the structure of the healthy ovary was clear, with good development of follicles at all levels, a clear granulosa cell layer in mature follicles, abundant follicular fluid, and intact oo-cytes visible. There was no obvious inflammatory cell infiltration or fibrosis in the ovarian stroma. In the NNOR group and NNOOgroup: no obvious morphological changes in the healthy ovary were observed. In the SNOR group and SNOO group: some structural ar-rangements in the healthy ovary were poor, with a reduction in the number of follicles at all levels and mature follicles, an increase inthe number of atretic follicles, and poor arrangement of granulosa cells. In the NOR group and NOO group: the former showed disor-dered arrangement of the healthy ovary structure, a significant reduction in the number of follicles at all levels, a significant increase inthe number of atretic follicles, a reduction and disordered arrangement of the granulosa cell layer, and visible vascular dilation and lym-phocytic infiltration in the ovarian stroma. The latter showed only partial poor arrangement of some ovarian structures in the healthy ova-ry. The levels of hormones (FSH, LH, AMH) between the NNOR group and the NNOO group, as well as between the SNOR group andthe SNOO group, showed no statistically significant differences (P > 0.05); when comparing the NOR group with the NOO group, the AMH level [(5 365.69±586.64) ng/L vs. (8 712.47±497.31) ng/L] was decreased and the FSH (588.46±55.55 vs. 431.23±36.32) and LH [(201.77±16.90) IU/L vs. (128.11±12.27) IU/L] levels were increased in the NOR group (P < 0.05); when comparing the hormone levelsin the CG group with those in the NNOR group, NNOO group, and NOO group, there were no statistically significant differences; howev-er, when compared with the SNOR group, SNOO group, and NOR group, the AMH level was lower and the FSH and LH levels werehigher in the CG group (P<0.05).Conclusion Torsion of the suspected necrotic ovary in the prematurity period has preservation val-ue, and the resection of the necrotic ovary has a protective effect on the healthy ovary in the sexual maturity period. |
