| 目的总结气性肾盂肾炎( EPN)的诊治经验，提高诊疗水平。方法回顾马鞍山市人民医院 2023年 8月 29日收治的 1例 EPN病人在住院期间的一般资料、临床表现、辅助检查及诊疗结果，并进行综合分析。结果 EPN是一种肾脏急性坏死感染性疾病，主要表现为肾实质弥漫性坏死，主要继发于基础疾病为糖尿病的病人，可导致脓毒症休克、多器官功能障碍综合征，预后极差，病死率极高。结论对于 EPN，CT是有效的影像诊断方法，治疗方案主要以综合治疗为主，强调早期诊断，强化抗感染治疗，及时外引流或内引流，必要时行肾切除术。 |

| Objective To summarize the diagnosis and treatment experience of emphysema pyelonephritis (EPN) and improve the lev-el of diagnosis and treatment.Method A comprehensive analysis was conducted on the general information, clinical manifestations,auxiliary examinations, and diagnosis and treatment results of a patient with EPN admitted to Ma'anshan People's Hospital on August29, 2023 during hospitalization.Results EPN was an acute renal necrosis infectious diseases, which was mainly manifested as diffusenecrosis of the renal parenchyma. The disease was mainly secondary to diabetes, which could lead to septic shock, multiple organ fail-ure, poor prognosis and high mortality.Conclusion CT is an effective imaging diagnostic method for this type of patient, and the treat-ment plan mainly focuses on comprehensive treatment, emphasizing early diagnosis, strengthening anti-infective treatment, timely exter-nal or internal drainage, and necessary nephrectomy. |
