      目的 探讨人软骨糖蛋白39(YKL-40)和单核细胞趋化蛋白1(MCP-1)水平与乳腺癌转移和预后的相关性。方法 采用酶联免疫吸附法分别检测64例乳腺癌和47例健康人群血清YKL-40和MCP-1水平,分析两项指标与乳腺癌临床病理的相关性以及与生存期和预后关系。结果 乳腺癌组患者YKL-40和MCP-1平均水平分别为(286.3±96.7 )ng·L-1和(171.2±105.4)μg·L-1,明显高于健康人群(164.1±70.9)ng·L-1和(118.5±77.6)μg·L-1(P<0.01);YKL-40升高与淋巴结转移有关(P<0.05),而MCP-1升高与有无淋巴结转移和TNM分期有关(P<0.05)。Kaplan-Meier分析显示YKL-40和MCP-1升高组3年生存率分别为46.75%和53.12%,而YKL-40和MCP-1正常组3年生存率分别为76.56%和81.25%(P<0.05)。Cox风险回归分析显示YKL-40和MCP-1均是判断乳腺癌预后的独立危险因素,而YKL-40升高对预后不良的相对危险度更高(RR=2.87 vs 2.25)。结论 血清YKL-40和MCP-1升高的乳腺癌患者淋巴结转移风险高。测定血清YKL-40和MCP-1水平,有助于判断乳癌患者预后。

      Objective To investigate the relationship between the serum cartilage glycoprotein 39 (YKL-40) and monocyte chemotactic protein-1 (MCP-1) with metastasis and prognosis in breast cancer patients.Methods The enzyme-linked immunosorbent method was carried out to detect the levels of serum YKL-40 and MCP-1 in 64 cases of breast cancer patients and 47 healthy people as control group.Analysis the relationship between indicators and clinical pathologic,survival and prognosis of breast cancer patients.Results The average value of YKL-40 and MCP-1 in Breast cancer patients were respectively (286.3±96.7) ng·L -1 and (171.2±105.4)μg·L -1,significantly higher than that of healthy people(164.1±70.9)ng·L -1and(118.5±77.6)μg·L -1 (P <0.01).The evaluated of YKL-40 was relate to lymph node metastasis (P<0.05),while the promotional of MCP-1 was associated with presence of lymph node metastasis and TNM stages (P<0.05).Kaplan-Meier analysis showed the 3 year survival rate in YKL-40 and MCP-1evaluated group were 46.75% and 53.12% respectively,has significant difference with the normal YKL-40 and MCP-1 groups(76.56% and 81.25%,respectively )(P<0.05).Cox regression analysis showed YKL-40 and MCP-1 were independent risk factors of prognosis of breast cancer,but the relative risk of YKL-40 was even higher than MCP-1(RR = 2.87 vs 2.25).Conclusion The breast cancer patients with elevated serum YKL-40 and MCP-1 have high risk of lymph node metastasis.It is helpful to judge the prognosis of patients with breast cancer by detect the serum levels of YKL-40 and MCP-1.
