      目的 研究胱抑素C(CysC)和视黄醇结合蛋白(RBP)浓度改变在检测胃癌化疗早期肾功能损伤中的临床意义。 方法 应用免疫比浊法分别检测第1周期和第2周期35例胃癌化疗患者和86例健康对照者血清CysC和RBP的浓度,同时检测血清肌酐(SCr)和尿素氮(BUN)的浓度。 结果 2个周期的化疗后,CysC浓度高于化疗前和对照组(P<0.05),第2周期CysC浓度高于第1周期同期(P<0.05)。RBP浓度高于化疗前和对照组(P<0.05),但第2周期与第1周期同期相比RBP浓度差异不明显(P>0.05)。BUN、Cr浓度仅在化疗第2周期后略有升高,与化疗前和对照组相比差异有显著性(P<0.01);与第1周期同期相比差异有显著性(P<0.05)。CysC和RBP含量变化与肾小球滤过率(GFR)变化呈负相关(P<0.05)。 结论 化疗导致早期肾功能损伤的患者中,血清CysC和RBP浓度明显升高。联合测定血清CysC和RBP有助胃癌化疗引起早期肾功能损伤的诊断,具有一定的临床应用价值。

      Objective To investigate the clinical significance of cystatin C(CysC)and retinol binding protein(RBP)concentration change in the evaluation of early renal function injury during chemotherapy in patients with gastric cancer. Methods The serum concentrations of CysC,RBP,creatinine(Cr)and blood urea nitrogen(BUN)were measured by immune turbidimetry in 35 patients with gastric cancer and 86 cases of healthy controls during the first and second cycle chemotherapies. Results The serum concentration of CysC after two-cycle chemotherapies was higher than that before chemotherapy and in the control group(P<0.05),and the second cycle was higher than the first cycle period(P<0.05).The serum concentration of RBP was higher than that before chemotherapy and the control group(P<0.05),but had no significant difference in the second cycle compared to the same period in the first cycle(P>0.05).The levels of BUN and Cr were increased slightly only after the second cycle chemotherapy,and had significant difference compared to before chemotherapy and the control group(P<0.01).The levels of BUN and Cr in the second cycle were significantly different compared with the same period in the first cycle(P<0.05).There was a negative correlation between the content of CysC and RBP and GFR(P<0.05). Conclusions The serum concentration of CysC and RBP were increased significantly in patients with early renal injury induced by chemotherapy,and is helpful for diagnosis of early renal injury caused by gastric cancer chemotherapy.The serum concentration of CysC and RBP has high value in clinical application.
