| 目的 探讨经直肠双平面超声及经体表高频超声在肛瘘诊断中的应用价值。 方法 分析 71 例分别采用经直肠双平面超声、经体表高频超声两种超声检查方式记录肛瘘的超声影像特征,并与手术结果对照,计算两种不同超声诊断方式诊断肛瘘主瘘管、分支瘘管、内口、外口与手术结果符合率,分析经直肠双平面超声及经体表高频超声的诊断优势。 结果 经体表高频组检查出肛瘘主瘘管、分支瘘管、内口、外口符合率分别为81.9％、68.5％、37.8％、100％,经直肠双平面组检查出肛瘘主瘘管、分支瘘管、内口、外口符合率分别为85.5％、71.1％、96.3％、0,两组比较,经体表高频超声在诊断肛瘘外口存在优势,经直肠双平面超声在定位瘘管内口、瘘管走形(主瘘管、分支瘘管)及与肛管周围括约肌关系存在优势,因此两种超声方法联合探查,取长补短,可全面诊断肛瘘所需信息,并提高了诊断准确率。 结论 经直肠双平面超声与经体表高频超声二者联合探查对于肛瘘的诊断具有很高的临床价值。 |

| Objective To discuss the application value of dual plane tranperineal sonography and the surface of high freqency utrasound in diagnosis of anal fistula. Methods Analyzed the sonography image of 71 patients whom was diagnosed with anal fistula operation by using high frequency ultrasound by the perineum and double plane ultrasound technology by transrectal,compared with intraoperative diagnosis of surgical doctors,calculated diagnose accordance rate of the main fistula,branch fistula,internal opening,enternal opening,the relationship of the main fistula with anal sphincter,compared with the surgical result.Advantages and disadvantages of high frequency ultrasound by the perineum and double plane ultrasound technology by transrectal were analyzed. Results The high frequency ultrasound by the perineum group check out diagnose accordance rate of the main fistula,branch fistula,internal opening,enternal opening,the relationship of the main fistula with anal sphincter were 81.9%,68.5%,37.8%,100%.The double plane ultrasound technology by transrectal group check out diagnose accordance rate of the main fistula,branch fistula,internal opening,enternal opening were 85.5%,71.1%,96.3%,0.The high frequency ultrasound by the perineum was more effective in the location of the enternal opening.The double plane ultrasound technology by transrectal was more effective in the location of the main fistula,branch fistula,internal opening.Around the advantages of two methods of ultrasonic joint detection,two methods of ultrasonic joint could make the best of the both worlds. Conclusions The combination of the two types of ultrasounds has very high diagnostic value in dignosis of anal fistula. |
