| 目的 探讨综合护理干预在老年糖尿病患者结肠镜诊疗前肠道准备中的应用价值。方法 前瞻性纳入2016年8~10月90例拟行结肠镜诊疗的老年糖尿病患者按照随机数字表法随机分为观察组和对照组,每组45例。两组均给予标准的肠道准备方案:聚乙二醇电解质散4 L分次方案,肠镜检查前1 d低纤维素饮食。其中,观察组从肠道准备重要性、饮食指导、降糖药物剂量调整等方面进行护理教育；对照组仅书面告知,未行护理教育、饮食指导和降糖药物剂量调整。对比两组患者耐受性、可接受度、不良反应、低血糖事件和肠道清洁质量。结果 观察组肠道准备的耐受性显著高于对照组[(4.5±1.0)分比(5.2±1.2)分,t=3.006,P=0.003],可接受度显著高于对照组(95.6%比73.3%,χ2=8.459,P=0.004),不良反应显著低于对照组(6.6%比22.2% ,χ2=4.406,P=0.036),低血糖事件发生率显著低于对照组(0%比15.6%,χ2=7.590,P=0.006),肠道清洁质量显著高于对照组(93.3%比75.6%,χ2=5.414,P=0.020)。结论 老年糖尿病患者结肠镜诊疗前针对肠道准备进行护理教育、饮食指导、降糖药物剂量调整等综合护理干预措施,可增强患者耐受性、降低不良反应,并提高肠道清洁质量。 |

| Objective To explore the application value of comprehensive nursing intervention in bowel preparation for colonoscopy in elderly patients with diabetes.Methods From August 2016 to October 2016,0 elderly patients with diabetes were randomly divided into the experimental group and the control group,45 cases in each group.The two groups were given standard bowel preparation scheme:polyethylene glycol electrolyte powder (PEG) 4 L scheme,and low fiber diet 1 day before enteroscopy.The experimental group was given nursing education,including the importance of intestinal preparation,diet guidance,hypoglycemic drug dosage adjustment,etc.The control group did not include an educational intervention.Patients in the control group were given written qualitative recommendations on a low-fiber diet for 1 day.The tolerability,acceptability,adverse reactions,hypoglycemic events and intestinal cleaning quality between the two groups were compared.Results The tolerability in the experimental group was significantly higher than the control group (4.5±1.0 vs.5.2±1.2,t=3.006,P=0.003).The acceptability was significantly higher than the control group(95.% vs. 73.3%,χ2=8.459,P=0.004).The adverse reaction was significantly lower than the control group 6.6% vs. 22.2 %,χ2=4.406,P=0.036),the incidence of hypoglycemia was significantly lower than the control group (0% vs. 15.6%,χ2=7.590,P=0.006),intestinal cleaning quality was significantly higher than the control group (93.3% vs. 75.6 %,χ2=5.414,P =0.020). Conclusion The comprehensive nursing intervention of bowel preparation for colonoscopy in elderly patients with diabetes,including nursing education,dietary guidance,and hypoglycemic drug dose adjustment,can improve patient tolerance,reduce adverse reactions,and improve the quality of intestinal cleaning. |
