| 目的 探讨经皮椎弓根螺钉治疗单阶段无神经损伤胸腰椎骨折的临床疗效。方法 回顾性研究2014年9月至2016年10月广安市广安区人民医院采用经皮椎弓根螺钉治疗单阶段无神经症状胸腰椎骨折25例,并与同期收治的28例开放手术病人相比较。结果 病人均获得随访,随访时间16~24个月,平均18.2个月,术前、术后伤椎前缘高度百分比和后凸Cobb's角,微创组分别为(63.06±1.50)%、(91.24±1.01)°、(24.10±1.49)%、(12.57±1.14)°；开放组分别为(62.95±1.36)%、(91.40±1.28)°、(23.82±1.81)%、(12.46±1.08)°,两组病例的术后伤椎前缘高度百分比、后凸Cobb's角等均较术前有显著改善(P<0.05),但两组间比较差异无统计学意义(P>0.05)。术后d1在腰背部疼痛VAS评分,微创组为(2.88±0.93)分,开放组为(3.43±0.57)分,微创组明显优于开放组(P<0.05)。首次下地负重时间,微创组为(12.92±1.73)d,开放组为(18.86±2.24)d,微创组早于开放组(P<0.05)。术中出血量、住院时间,微创组分别为(54.20±6.40)mL、(8.08±1.82)d,开放组分别为(07.14±11.90)mL、(14.64±1.91)d,微创组较开放组明显减少(P<0.05)。结论 经皮椎弓根螺钉治疗单纯胸腰椎骨折疗效肯定,病人术中出血少,下地负重时间早,疼痛缓解明显,值得临床推广和利用。 |

| ObjectiveTo explore the clinical effect of percutaneous pedical screw fixation for the treatment of single segment thoracolumbar fracture without neural injury.Method From September 2014 to October 2016,data of 25 cases with percutaneous pedical screw fixation were retrospectively analyzed,comparing with 28 cases with open pedicle screw fixation.Results All cases were followed up,18.2 months in average (range 16~24).Preoperative and postoperative vertebral height percent and Cobb's angle of minimally invasive surgery group were (63.06±1.50)%,(91.24±1.01)°,(24.10±1.49)%,(12.57±1.14)°,of open surgery group were (62.95±1.36)%,(91.40±1.28)°,(23.82±1.81)%,(12.46±1.08)°.Postoperative vertebral height percent and Cobb's angle were promoted obviously after surgery (P<0.05),with no significant different in vertebral height percent and Cobb's angle between preoperation and postoperation in each group (P>0.05).VAS score in one day after surgery of minimally invasive surgery group (2.88±0.93) were significantly better than that of open surgery group (3.43±0.57).For time to weight bearing,amount of bleeding,duration of hospital stay,minimally invasive surgery group were (12.92±1.73) d,(54.20±6.40) mL,(8.08±1.82) d,open surgery group were (18.86±2.24) d,(107.14±11.90) mL,(14.64±1.91) d.Comparing with the open surgery group,minimally invasive surgery group significantly reduced time to weight bearing,the amount of bleeding,length of hospital stay,and made VAS score better results in one day after surgery.Conclusion Percutaneous pedicle screw fixation has a certain effect for the treatment of thoracolumbar fracture with less bleeding,early load time,obviously pain relief and is worthy in the clinical promotion. |
