| 目的 提高静脉用药调配中心(PIVAS)医嘱审核水平,促进临床合理用药。方法 收集重庆市大足区人民医院PIVAS 2015年5月至2017年12月的不合理医嘱,并进行统计分析。结果 不合理用药医嘱2 284份,占全部医嘱的0.90‰。不合理用药医嘱类型主要为溶媒不适宜、给药浓度不合理、配伍禁忌、给药剂量不合理、药品遴选不适宜。不合理用药医嘱率呈逐年下降趋势。结论 PIVAS药师审核医嘱,可明显提高静脉用药的安全性、合理性、有效性。该院PIVAS药师医嘱审核能力有待进一步提高。 |

| Objective To improve the level of reviewing doctor's advice in Pharmacy Intravenous Admixture Service (PIVAS),so as to promote rational use of drugs in clinic.Methods Irrational doctor's advice in PIVAS of The People's Hospital of Dazu District in Chongqing from May 2015 to December 2017 was collected and statistically analyzed.Results There were 2 284 pieces of irrational doctor's advice with the rate of 0.90‰,which mainly contained improper solvent,irrational concentration,incompatibility,irrational dose,and improper drug selection.The rate of irrational doctor's advice showed a downward trend year by year.Conclusion That pharmacists in PIVAS reviewed doctor's advice can obviously improve the safety,rationality and effectiveness of intravenous drugs.And pharmacists' ability to review doctor's advice should be further improved. |
