      目的观察依帕司他联合生物电刺激治疗糖尿病周围神经病变的疗效。方法选择 2017年 1月至 2018年 12月在赤峰学院附属医院诊治的糖尿病周围神经病变病人 90例作为研究对象,根据随机数字表法,将其分为依帕司他组,生物电刺激组(TENS组)和联合治疗组,每组各 30例。三组均进行 4周治疗,依帕司他组以 50毫克 /次, 3次/天剂量口服依帕司他,生物电刺激组以 8分钟 /次, 2次/天频率进行经皮电神经刺激,联合组采取两者结合治疗,用法用量与上述相同,随访 1周。观察并比较各组疗效,检测并比较各组治疗前后血清超氧化物歧化酶(SOD)、丙二醛、 8?羟基脱氧鸟苷酸(8?OHdG)水平、震动感觉阈值和神经电生理情况。结果依帕司他组有效率 73.3%,TENS组有效率 70.0%,联合治疗组有效率 96.7%,三组间差异有统计学意义(χ2=7.917,P=0.019)。治疗前三组血清 SOD、丙二醛、 8?OHdG水平、腓总神经、正中神经传导速度和 VPT数值差异无统计学意义(P>0.05)治疗后三组 SOD水平、腓总神经及正中神经传导速度明显增加,丙二醛、8?OHdG水平及 VPT数值明显降低(均 P<0.05),且以联,合治疗组改善程度最为明显(P<0.016 7)。结论依帕司他联合生物电刺激治疗糖尿病周围神经病变可明显改善症状和神经传导速度,提高机体抗氧化能力,具有较高临床应用价值。

      Objective To observe the efficacy of epalrestat combined with bioelectric stimulation in the treatment of diabetic pe? ripheral neuropathy.Methods Ninety patients with diabetic peripheral neuropathy in the Affiliated Hospital of Chifeng Universityfrom January 2017 to December 2018 were selected as the research objects.According to the random number table method,theywere assigned into epalrestat group,bioelectrical stimulation group(TENS group)and combined treatment group,30 cases in each group.All three groups were treated for 4 weeks.Epalrestat group was given epalrestat 50 mg/time,3 times/day,while bioelectrical stimulation group was given percutaneous electrical nerve stimulation at the frequency of 8 minutes/time and 2 times/day.The com?bination group was treated with the combination of the two,with the same usage and dosage,and the follow?up was 1 week.The cu?rative effect of each group was observed and compared.The levels of serum superoxide dismutase(SOD),malondialdehyde(MDA), 8?hydroxydeoxyguanosine acid(8?OHdG),vibration sensation threshold and neuroelectrophysiology were detected and compared in each group before and after treatment.Results The effective rate in epalrestat group,TENS group and combined treatment group was 73.3%,70.0% and 96.7%,respectively,and the difference was statistically significant between the three groups(χ2=7.917,P=0.019).There was no significant difference in serum SOD,MDA,8?OHdG,conduction velocity of common peroneal nerve,median nerve and VPT among the three groups before treatment(P>0.05).After treatment,SOD level,conduction velocity of common pero? neal nerve and median nerve increased significantly,while malondialdehyde,8?OHdG and VPT decreased significantly(all P<0.05),and the improvement of combined treatment group was the most obvious(P<0.016 7).Conclusion Epalrestat combinedwith bioelectrical stimulation in the treatment of diabetic peripheral neuropathy can significantly improve the symptoms and nerveconduction velocity,improve the body’s antioxidant capacity,and has high clinical application value.
