      目的探讨呼吸道合胞病毒( RSV)下呼吸道感染肺炎病儿外周血血浆中微小 RNA-145(mir-145)表达与外周血单个核细胞( PBMC)中 T淋巴细胞亚群的关系。方法选取 2017年 1月至 2018年 12月海南现代妇女儿童医院收治的 RSV下呼吸道感染肺炎病儿 126例作为 RSV肺炎组,同期在本院进行体检的健康儿童 130例作为健康对照组。 RSV肺炎组根据病情严重程度分为轻度组( n = 54)、重度组( n = 72)并依据重度组是否给予机械通气分为非机械通气组( n = 38)、机械通气组( n = 34)。利用实时荧光定量 PCR(qRT-PCR)检测外周,血血浆中 mir-145表达水平,使用流式细胞仪及配套试剂盒检测 PBMC中 T淋巴细胞亚群 CD3+CD4+CD8+水平,并用 MRFlow流式细胞分析软件自动行 CD3+CD4+CD8+绝对计数,同时计算 CD4+/CD8+值。结RSV肺炎组、病儿、血浆 mir-145表达[(1.74±0.48)比( 0.99±0.26)]及PBMC中、CD8+、水平[(30.75±8.39)%比( 21.64±5.92)%]均显果著高于健康对照组( P < 0.05)PBMC中 CD3+[( 59.04±16.12)%比( 68.77±18.35)%]、 CD4+水平[( 31.27±8.01)%比( 46.90±12.84)%]及 CD4+/CD8+值[( 1.02±08)比( 2.17±0.63)]均明显低于健康对照组( P < 0.05)。重度组病儿血浆 mir-145表达.2,[(1.87±0.55)比( 1.52±0.40)]及 PBMC中 CD8+水平[(33.96±9.10)%比( 25.34±7.02)%]均明显高于轻度组病儿( P < 0.05)PBMC中 CD3+[( 57.05±15.83)%比( 63.28±17.21)%]、 CD4+水平[( 23.78±6.62)%比( 38.09±10.14)%]及 CD4+/CD8+值[( 0.70±0.2,1)比(1.50±0.39)]均显著低于轻度组病儿( P < 0.05)。重度 RSV肺炎机械通气组病儿血浆 mir-145表达[( 1.95±0.58)比( 1.68±0.45)]及PBMC中 CD8+水平[( 34.92±9.18)%比( 28.17±7.49)%]均显著高于非机械通气组病儿( P < 0.05), PBMC中 CD3+[( 54.72±13.53)%比( 61.83±16.04)%]、 CD4+水平[( 20.45±5.79)%比( 34.03±8.85)%]及 CD4+/CD8+值[( 0.59±0.17)比( 1.21±0.36)]均明显低于非机械通气组病儿( P < 0.05)。RSV肺炎病儿血浆 mir-145表达与 PBMC中 CD3+水平呈负相关( P < 0.05)PBMC中 CD4+水平呈负相关( P < 0.05)与 PBMC中 CD8+水平呈正相关( P < 0.05),与 CD4+/CD8+值呈负相关(P<0.05)。结论与,RSV下呼吸道感染肺炎病儿外周血血浆中,mir-145表达水平升高,其高表达可能与 T淋巴细胞亚群异常有关。

      Objective To investigate the relationship between the expression of microRNA-145 (mir-145) in peripheral blood plasma and T lymphocyte subsets in peripheral blood mononuclear cells (PBMC) of children with respiratory syncytial virus (RSV) pneumonia.Methods One hundred and twenty-six children with RSV pneumonia admitted to Hainan Modern Women and Children's Hospital from January 2017 to December 2018 were selected as the RSV pneumonia group, and 130 healthy children who underwent physicalexamination at the same time were selected as the control group. The children in RSV pneumonia group were assigned into mild group(n = 54) and severe group (n = 72) according to the severity of the disease, and the children in severe group were assigned into non-mechanical ventilation group (n = 38) and mechanical ventilation group (n = 34). Real-time fluorescence quantitative PCR (qRT-PCR) was used to detect the expression of miR-145 in peripheral blood plasma, the levels of CD3+, CD4+, CD8+ of T lymphocyte subsets in PBMC were detected by flow cytometry and kits, the absolute counts of CD3+, CD4+, CD8+ were automatically performed by MRFlow flow cytometry software, at the same time, the value of CD4+/CD8+ was calculated.Results The expression of plasma miR-145 [(1.74±0.48) vs. (0.99±0.26)] and CD8+ [(30.75±8.39)% vs. (21.64±5.92)%] in PBMC in RSV pneumonia group were significantly higher than those in control group (P < 0.05). The levels of CD3+ [(59.04±16.12)% vs. (68.77±18.35)%], CD4+ [(31.27±8.01)% vs. (46.90±12.84)%] and CD4+/CD8+[(1.02±0.28) vs. (2.17±0.63)] in PBMC were significantly lower than those in control group (P < 0.05). The levels of plasma miR-145 [(1.87±0.55) vs. (1.52±0.40)] and CD8+[(33.96±9.10)% vs. (25.34±7.02)%] in PBMC in severe group were significantly higher than those in mild group (P < 0.05), while the levels of CD3+ [(57.05±15.83)% vs. (63.28±17.21)%], CD4+ [(23.78±6.62)% vs. (38.09± 10.14)%] and CD4+/CD8+ [(0.70±0.21) vs. (1.50±0.39)] in PBMC were significantly lower than those in mild group (P < 0.05). The plasma miR-145 expression [(1.95±0.58) vs. (1.68±0.45)] and CD8+ level [(34.92±9.18)% vs. (28.17±7.49)%] in PBMC of children with severe RSV pneumonia in mechanical ventilation group were significantly higher than those in non-mechanical ventilation group (P < 0.05), while the levels of CD3+[(954.72±13.53)% vs. (61.83±16.04)%], CD4+[(20.45±5.79)% vs. (34.03±8.85)%] and CD4+/CD8+[(0.59± 0.17) vs. (1.21±0.36)] in PBMC were significantly lower than those in non-mechanical ventilation group (P < 0.05). The expression of miR-145 in plasma of children with RSV pneumonia was negatively correlated with CD3+ level in PBMC (P < 0.05), negatively correlated with CD4+ level in PBMC (P < 0.05), positively correlated with CD8+ level in PBMC (P < 0.05), and negatively correlated with CD4+/CD8+ level in PBMC (P < 0.05).Conclusion The expression of miR-145 in peripheral blood plasma of children with RSV pneumonia is elevated, and the high expression of miR-145 may be related to abnormal T lymphocyte subsets.
