      目的探究嗜酸性粒细胞( EOS)、白细胞介素 -23(IL-23)/Th17轴及小气道功能与小儿咳嗽变异性哮喘( CVA)的相关性。方法选取徐州市儿童医院于 2017年 5月至 2019年 5月收治的 150例 CVA病儿,依照哮喘严重程度分为 1~2级组(对照组, n=81)与 3~4级组(研究组, n=69)另选择同期来该院健康检查的健康儿童 50例作为健康对照组,对比三组儿童外周血 EOS、血清白细胞介素 -17(IL-17)、 IL-23,水平及小气道功能[肺 25%、50%及 75%肺活量的最大呼气量( FEF25%,FEF50%, FEF75%)及最大呼气中流速( FEF25%~75%)]并分析各指标相关性。结果三组儿童外周血 EOS、血清 IL-17、IL-23水平及小气道功能比较差异有统计学意义( P<0.05)。,其中,研究组外周血 EOS(13.6±2.3)%、血清 IL-17(20.2±3.8)ng/L、IL-23水平(17.7±4.5)ng/L显著高于健康对照组[( 0.7±0.2)%、(3.5±0.6)ng/L和( 8.2±2.6)ng/L)]和对照组[( 8.5±1.5)%、(13.2±3.0)ng/L和(10.4±2.8)ng/L](P<0.05)且研究组的 FEF25%、FEF50%、FEF75%及 FEF25%~75%[( 64.6±6.0)L/s、(44.2±6.8)L/s、(26.2±5.1) L/s和( 29.0±5.7)L/s]均显著低,于健康对照组[(92.5±12.6)L/s、(92.9±13.4)L/s、(73.4±9.7)L/s和( 80.6±9.5)L/s]和对照组[(80.2±10.9)L/s、(84.4±9.3)L/s、(58.1±8.8)L/s和( 68.4±7.6)L/s](P<0.05)。对比三组病儿的 PEF、PIF、FEV1、FIV1、FEF50、FIF50,发现均差异有统计学意义( P<0.05)。 Pearson相关性分析结果显示, EOS与 IL-17、IL-23炎症指标均呈正相关( r=0.598,P<0.001;r=0.742,P<0.001),而 EOS、IL-17、IL-23与气道功能指标均呈负相关( P<0.05)。结论 EOS、IL-17及 IL-23水平在小儿咳嗽变异性哮喘中存在明显相关性, EOS与 IL-17、IL-23均呈正相关,可显著降低病儿小气道功能,对于 CVA疾病发展中具有重要作用。

      Objective To investigate the relationship between eosinophils (EOS), interleukin-23 (IL-23)/Th17 axis and small airway function and cough variant asthma (CVA) in children. Methods A total of 150 children with chronic cough admitted to Xuzhou Chil?dren′s Hospital from May 2017 to May 2019 were enrolled. Among them, 69 patients with CVA were diagnosed as the study group, and81 patients were selected as the control group. 50 healthy children in the hospital health check were used as healthy control group. TheEOS, serum IL-17, IL-23 levels of peripheral blood and small airway function [FEF25%, FEF50%, FEF75% and FEF25%-75%] were compared between the three groups, and the correlation of each index was analyzed. Results There were statistically significant differ? ences in the levels of peripheral blood EOS, serum IL-17, IL-23 and small airway function among the three groups of children (P<0.05).Among them, the peripheral blood EOS (13.6±2.3)%, serum IL-17 (20.2±3.8)ng/L, and IL-23 levels (17.7±4.5)ng/L of the study groupwere significantly higher than those of the healthy control group [(0.7±0.2)%, (3.5±0.6)ng/L and (8.2±2.6)ng/L)] and the control group[(8.5±1.5)%, (13.2±3.0)ng/L and (10.4±2.8)ng/L] (P<0.05), and the FEF25%, FEF50%, FEF75% and FEF25%-75% of the study group[( 64.6±6.0)L/s, (44.2±6.8)L/s, (26.2±5.1)L/s and (29.0±5.7)L/s] were all significant Lower than the healthy control group [(92.5±12.6)L/s, (92.9±13.4)L/s, (73.4±9.7)L/s and (80.6±9.5)L/s] and the control group [(80.2±10.9)L/s, (84.4±9.3)L/s, (58.1±8.8)L/s and (68.4±7.6)L/s] (P<0.05). The PEF, PIF, FEV1, FIV1, FEF50, FIF50 of the three groups of children were compared, and the differences were found to be statistically significant (P<0.05).The results of Pearson correlation analysis showed that the severity of EOS was positively correlat? ed with IL-17 and IL-23 inflammation indicators (r=0.598, P<0.001; r=0.742, P<0.001), but EOS、IL-17、IL-23 and airway function were negatively correlated (P<0.05).Conclusion Eosinophils, IL-23 and IL-23 levels play an important role in the development of cough variant asthma in children, which can significantly reduce the small airway function of children, leading to increased disease.
