      目的探讨老年人认知损害与睡眠障碍的关系并且分析引起认知损害的影响因素,为改善老年人睡眠质量及认知功能提供参考。方法 2016年 1月至 2017年 12月,石家庄市第八医院精二科门诊及病房,通过方便取样对 152例伴有失眠的老年人进行问卷调查,并另选取 150例健康志愿者(睡眠正常组)进行比较。采用匹茨堡睡眠质量指数量(PSQI)表、蒙特利尔认知评估(MoCA)评分表分别评估老年人的睡眠质量以及认知功能, Spearman相关系数法分析睡眠障碍与认知损害之间的关系,多因素 logistic回归分析不同维度睡眠障碍对认知损害的影响。结果 152例睡眠障碍者的 PSQI的平均得分为(14.67±1.68)分;其 MoCA平均得分(16.79±3.89)分;相关分析结果表明, MoCA总分与各子项均与 PSQI总分负相关;多因素 logistic回归分析表明,睡眠时间、入睡时间以及日间功能障碍与发生认知损伤差异有统计学意义(P<0.05)。结论睡眠障碍的老年人中认知损害发生率增加,其中合理的睡眠时间、入睡时间可改善认知功能,对认知功能起保护作用;而日间功能障碍与认知损伤相互影响,可导致病情加重。

      Objective To explore the relationship between cognitive impairment and sleep disorder in the elderly and analyze the influencing factors of cognitive impairment, so as to provide reference for improving sleep quality and cognitive function in the elderly.Methods One hundred and fifty-two seniors with insomnia in the Outpatient and Ward of the Second Department of Spermatology inthe Eighth Hospital of Shijiazhuang City from January 2016 to December 2017 were investigated by convenient sampling, and 150healthy volunteers with normal sleep were selected as normal group. The sleep quality and cognitive function of the elderly were assessed by Pittsburgh Sleep Quality Index (PSQI) and Montreal Cognitive Assessment (MoCA). Spearman correlation coefficient methodwas used to analyze the relationship between sleep disorders and cognitive impairment. Multivariate logistic regression was used to analyze the effects of different sleep disorders on cognitive impairment.Results The average PSQI score of 152 patients with sleep disorders was (14.67±1.68), and the average MoCA score was (16.79±3.89). The results of correlation analysis showed that the total score ofMoCA and its subitems were negatively correlated with the total score of PSQI. Multivariate logistic regression analysis showed that cognitive impairment was statistically different from the length of sleep, time of fall asleep and daytime dysfunction.Conclusions The incidence of cognitive impairment in the elderly with sleep disorders is increasing. Reasonable sleep time and falling asleep time can improve cognitive function and protect cognitive function; while daytime dysfunction and cognitive impairment interact with each otherand can lead to illness aggravate.
