| 目的探讨三角韧带损伤修复对踝关节骨折切开复位内固定术后踝关节内侧间隙和关节外翻的影响。方法建湖县中医院 2016年 1月至 2019年 1月期间收治 120例踝关节骨折，采用随机数字表法分为观察组和对照组，每组病人 60例，两组病人均采取切开复位内固定手术，观察组病人在此基础上采取三角韧带修复手术。比较两组踝关节功能恢复、疼痛情况、主动外翻角以及外翻力度、踝关节内侧间隙之间的差异。结果与对照组相比，观察组踝关节功能恢复情况显著好于对照组(对照组良好率 55%，观察组 75%，P<0.05)踝关节主动外翻角显著低于对照组，(24.05±0.80)°比( 28.68±0.76)°，P<0.001，外翻力度显著高于对照组，(482.05±10.80)N比(4，53.68±10.76)N，P<0.001；两组踝关节内侧间隙均显著下降，且观察组踝关节内侧间隙显著低于对照组，(2.49±1.22)mm比( 3.12±1.43)mm，P<0.05。结论三角韧带损伤修复手术对踝关节骨折切开复位内固定术后踝关节内侧间隙和关节外翻显著改善，功能恢复良好，建议临床推广。 |

| Objective To explore the effect of deltoid ligament injury repair on medial space and valgus of ankle after open reduction and internal fixation of ankle fracture.Methods The subjects of this study were 120 patients with ankle fractures treated in Jianhu Hospital of Traditional Chinese Medicine from January 2016 to January 2019 who were randomly assigned into observation group and controlgroup, 60 patients in each group. Both groups were treated with open reduction and internal fixation. On the basis of this, patients in theobservation group underwent triangular ligament repair. A comparison was made of ankle function recovery, pain, active valgus angle, valgus strength, and medial space of the ankle joint between the two groups.Results Compared with the control group, the recovery of ankle joint function was significantly better (75% vs. 55%, P<0.05), the active valgus angle of ankle joint was significantly lower (24.05±0.80)° vs. (28.68±0.76)°,P<0.001, and the valgus force was significantly stronger ,(482.05±10.80)N vs. (453.68±10.76)N,P<0.001,in the observation group. The medial space of ankle joint in the two groups was both significantly smaller, and the medial space of ankle joint in theobservation group was significantly smaller than that in the control group [(2.49±1.22)mm vs. (3.12±1.43)mm, P<0.05.Conclusion The repair operation for deltoid ligament injury can significantly improve the medial space and joint valgus of ankle joint after open reductionand internal fixation of ankle fracture, and restore the function of the ankle, which is recommended to be popularized. |
