| 目的探讨解毒泄浊益肾汤联合苯溴马隆对痛风性关节炎病人血清 3-硝基酪氨酸( 3-NT)水平、 Lysholm评分及疼痛数字评价量表( NRS)评分的影响。方法回顾性分析 2016年 4月至 2018年 4月武汉市第五医院 80例痛风性关节炎门诊病人临床资料。依据治疗方法不同分为对照组 40例和观察组 40例。治疗前，两组病人均进行低嘌呤饮食，大量饮水，使尿量维持在 2 000～3 000 mL/d。对照组给予苯溴马隆治疗，观察组在对照组基础上给予解毒泄浊益肾汤治疗。通过酶联免疫吸附法(ELISA)测量血清 8-羟基脱氧鸟苷( 8-OHdG)、 3-NT水平、白细胞介素 -6(IL-6)、白细胞介素 -10(IL-10)、白细胞介素 -1β(IL1β)、肿瘤坏死因子 -α(TNF-α)水平；通过全自动生化分析仪尿酸酶测定血清尿酸(BUA)水平；通过 Lysholm系统评估治疗前后膝关节功能；通过 NRS评估治疗前后膝关节疼痛程度，并观察不良反应发生情况。结果治疗后，观察组治疗总有效率为 97.50%，对照组为 80.00%，经秩和检验，差异无统计学意义( P>0.05)。治疗前，两组血清 8-OHdG、3-NT、BUA、IL-6、IL-10、IL1β、TNF-α、Lysholm、NRS比较，差异无统计学意义( P>0.05)。治疗后，观察组和对照组血清 8-OHdG、3-NT、BUA、IL-6、IL-10、 IL-1β、TNF-α、NRS均低于治疗前，观察组低于对照组( P<0.05)；治疗后 Lysholm评分，观察组［( 95.68±26.68)分比( 44.39± |

| Objective To explore the effects of Jiedu Xiezhuo Yishen decoction combined with benzbromarone on serum 3-nitrotyrosine (3-NT), Lysholm score and numerical rating scale (NRS) score in patients with gouty arthritis.Methods The clinical data of 80 outpatients with gouty arthritis treated in The Fifth Hospital in Wuhan from April 2016 to April 2018 were retrospectively analyzed.They were assigned into control group (40 cases) and experimental group (40 cases) according to different treatment methods. Beforetreatment, both groups were fed a low purine diet and drank plenty of water to maintain urine volume between 2 000-3 000 mL/d. The control group was treated with benzbromarone, while the experimental group was treated with Jiedu Xiezhuo Yishen decoction on the basis of the control group. The levels of serum 8-hydroxydeoxyguanosine (8-OHdG), 3-NT, interleukin-6 (IL-6), interleukin-10 (IL-10), interleukin-1β (IL-1β) and tumor necrosis factor (TNF-α) were measured by enzyme-linked immunosorbent assay (ELISA), the level of serum uric acid (BUA) was measured by uricase-free biochemical analyzer, knee function was assessed by Lysholm system before and after treatment; and knee pain was assessed by NRS before and after treatment, the adverse reactions were observed.Results After treatment, the total effective rate of gouty arthritis patients in the experimental group was 97.50%, and that in the control group was 80.00%,the difference was not statistically significant by rank sum test (P>0.05). Before treatment, there was no significant difference in serum 8-OHdG, 3-NT, BUA, IL-6, IL-10, IL-1β, TNF-α, Lysholm and NRS between the two groups (P>0.05). After treatment, the serum 8OHdG, 3-NT, BUA, IL-6, IL-10, IL-1β, TNF-α, NRS of gouty arthritis patients in the experimental group and the control group werelower than those before treatment, and these indexes in the experimental group were lower than those in the control group (P<0.05); the Lysholm scores of gouty arthritis patients in the experimental group [(95.68±26.68) vs. (44.39±17.93)] and the control group [(83.54± 23.37) vs. (44.28±17.85)] were higher than those before treatment, while that in the experimental group was higher than that in the control group (P<0.05); there was no significant difference in the incidence of adverse reactions between the experimental group and the control group (P>0.05).Conclusion Jiedu Xiezhuo Yishen decoction combined with benzbromarone can effectively improve the oxidative stress state, reduce the level of serum uric acid, inhibit the occurrence of inflammatory reaction, and effectively relieve knee painand restore knee function, which has certain popularization value. |
