      目的探讨未合并慢性肾脏病( CKD)的不同严重程度阻塞性睡眠呼吸暂停综合征( OSAS)病人血清胱抑素 C(Cys-C)水平及其影响因素,为早期发现及诊治 OSAS病人肾功能损害提供依据。方法选取 2019年 1―12月在徐州医科大学附属医院就诊的 OSAS病人 150例,均进行多导睡眠图( PSG)检查,采集空腹血样测定 Cys-C、肌酐、尿酸、超敏 C反应蛋白( hsCRP)等,依据呼吸暂停低通气指数( AHI)分为对照组 30例( AHI<5)、轻度组 40例( AHI≥5~15)、中度组 35例( AHI≥15~30)、重度组 45例( AHI≥30),比较四组血清肌酐、肾小球滤过率( eGFR)、 Cys-C浓度并分析其相关因素,评价 OSAS病人肾损伤情况。结果四组肌酐、尿素、 eGFR差异无统计学意义( P>0.05),与对照组相比,重度组血清 Cys-C水平[(0.96±0.31)mg/L比( 0.64±0.10)mg/

      Objective To study the serum Cys-C level and its influencing factors in patients with obstructive sleep apnea syndrome(OSAS) of different severity without chronic kidney disease (CKD), so as to provide reference for early diagnosis and treatment of renaldysfunction in patients with OSAS.Methods A total of 150 patients with OSAS in the Affiliated Hospital of Xuzhou Medical Universi.ty form January to November 2019 were selected. All patients underwent polysomnography (PSG), and fasting blood samples were col.lected to determine Cys-C, creatinine, urea, hsCRP, etc. According to the apnea hypopnea index (AHI), patients were divided into fourgroups: 30 cases in the control group (AHI<5), 40 cases in the mild group (AHI≥5-15), 35 cases in the moderate group (AHI≥15-30) and 45 cases (AHI≥30) in the severe group to evaluate the renal injury in patients with OSAS.Results There were no statistically sig. nificant differences in Cr, urea, and eGFR among the four groups (P>0.05). Compared with the control group, serum Cys-C [(0.96±0.31) mg/L vs. (0.64±0.10) mg/L], BMI [(35.39±8.89) kg/m2 vs. (25.12±4.86) kg/m2], neck circumference [(42.4±5.32) cm vs. (37.73±3.37) cm], chest circumference [(113.56±15.32) cm vs. (106.11±11.56) cm], abdominal circumference [(115.22±18.49) cm vs. (103.87±11.78) cm], hsCRP, the longest apnea time (LAT) increased (P<0.05). ASpO2 and MSpO2 were lower in the severe group than those in the con. trol group [(86.20±6.17) % vs. (95.07±1.70) %,(66.93±13.98) % vs. (86.37±5.50) %] and mild to moderate group (P<0.05). BMI, AHI, LAT were positively correlated with CysC (r>0, P<0.05). There was significant negative correlation between ASpO2, MSpO2 and CysC (r<0, P< 0.05).Conclusions As a sensitive indicator of renal damage, serum Cys-C is related to the severity of OSAS. When creatinineand glomerular filtration rate are not abnormal, OSAS patients have started the process of renal damage.
