      目的探讨缺血性脑白质病变( WMIL)病人血清中基质金属蛋白酶 -3(MMP3)、基质金属蛋白酶 -9(MMP9)水平与 WMIL严重程度的相关性。方法连续选取 2019年 5月至 2020年 5月安徽医科大学附属宿州医院神经内科收住的 WMIL病人 117例及同时期该院的健康体检者 98例,根据 Fazekas分级量表将 WMIL病人分为轻度组(1~4分)、中度组( 5~8分)、重度组(9~ 12分)。收集受检者的一般临床资料。采用酶联免疫吸附测定检测受检者血清中 MMP3、MMP9水平。比较指标差异性。结论WMIL组糖尿病患病率 64.10%、总胆固醇( 5.15±0.98)mmol/L、同型半胱氨酸( HCY)(17.16±4.96)mmol/L、半胱氨酸蛋白酶抑果制剂 C(以下简称“胱抑素 C”)(1.41±0.33)mg/L、MMP3(201.95±36.19)μg/L、MMP9(60.41±19.21)μg/L水平均高于健康对照组糖尿病患病率 31.63%、总胆固醇( 4.90 ±0.73)mmol/L、HCY(12.25±2.06)mmol/L、胱抑素 C(0.90±0.25)mg/L、MMP3(119.34±21.29)μg/L、MMP9(25.47±5.81)μg/L,差异有统计学意义( P<0.05);不同严重程度 WMIL各组血清中 MMP3[(155.22±20.27)μg/ L、(218.73±14.55)μg/L、(221.63±21.01)μg/L]和 MMP9[( 35.25±6.65)μg/L、(76.99±9.58)μg/L、(110.07±14.72)μg/L]水平差异有统计学意义( P<0.05);多因素 logistic回归分析显示 HCY(OR=0.119,95%CI:0.024~0.597,P=0.008)、 MMP3(OR=1.099, 95%CI:1.057~1.143,P<0.001)、 MMP9(OR=1.311,95%CI:1.138~1.509,P<0.001)是 WMIL病人的独立危险因素( P<0.05);根据 Spearman相关性分析得出 MMP3、MMP9水平与 WMIL严重程度呈正相关( r=0.786、0.866,P<0.05)。结论 WMIL病人血清中 MMP3、MMP9水平与疾病严重程度呈正相关, HCY、MMP3、MMP9是 WMIL的独立危险因素。

      Objective To investigate the correlation between serum levels of matrix metalloproteinase-3 (MMP3) and matrix metalloproteinase-9 (MMP9) in patients with white matter ischemic lesions (WMIL) and the severity of WMIL.Methods A total of 117 WMIL patients diagnosed and 98 healthy people in the Department of Neurology, Suzhou Hospital Affiliated to Anhui Medical University fromMay 2019 to May 2020 were selected. WMIL patients were divided into a mild group (1-4 points), moderate group (5-8 points), and severe group (9-12 points) according to the Fazekas scale. The general clinical data of the subjects were collected. The serum levels ofMMP3 and MMP9 were detected by enzyme-linked immunosorbent assay (ELISA), and the differences in the indicators were compared. Results The prevalence of diabetes (64.10%) and the levels of total cholesterol (5.15±0.98) mmol/L, homocysteine (HCY, 17.16±4.96) mmol/L, cystatin C (1.41±0.33) mg/L, MMP3 (201.95±36.19) μg/L and MMP9 (60.41±19.21) μg/L in the WMIL group were higher than those in the healthy control group [prevalence of diabetes (31.63%), total cholesterol (4.90±0.73) mmol/L, HCY (12.25±2.06)mmol/L, cystatin C (0.90±0.25) mg/L, MMP3 (119.34±21.19) μg/L, MMP9 (25.47±5.81) μg/L, and the difference was statistically significant (P < 0.05). Serum levels of MMP3 [(155.22±20.27) μg/L, (218.73±14.55) μg/L, (221.63±21.01) μg/L] and MMP9 [(35.25±6.65) μg/L, (76.99±9.58) μg/L, (110.07±14.72) μg/L] were significantly different among the WMIL groups (P < 0.05). Multivariate logistic regression analysis showed that HCY (OR=0.119, 95%CI: 0.024-0.597, P = 0.008), MMP3 (OR=1.099, 95% CI:1.057-1.143, P < 0.001), and MMP9 (OR=1.311, 95% CI:1.138-1.509, P < 0.001) were independent risk factors for WMIL patients (P < 0.05).
