      目的探讨肠易激综合征(IBS)病人血清连蛋白(Zonulin)、单核细胞趋化蛋白-1(MCP-1)水平与肠道菌群、炎症反应及胃肠症状严重程度的相关性。方法选取2017年2月至2020年3月云南省第三人民医院收治的IBS病人92例,分为便秘型IBS(IBS-C)35例(IBS-C组),腹泻型IBS(IBS-D)57例(IBS-D组)。同期选取50例健康志愿者为对照组,检测血清Zonulin、MCP-1,白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)水平,对肠道双歧杆菌、乳杆菌、肠杆菌、肠球菌进行培养,并计算菌含量,采用IBS临床症状严重程度评分系统(IBS-SSS)评估IBS严重程度。分析Zonulin、MCP-1与肠道菌群、炎性因子及胃肠症状严重程度的相关性。结果IBS-D组血清MCP-1水平[(313.76±77.93)ng/L比(186.52±33.56)ng/L、(268.52±52.65)ng/L]、肠杆菌[(9.36±0.81)CFU/g 比(7.42±0.83)CFU/g、(8.18±0.92)CFU/g]及肠球菌含量[(8.23±1.21)CFU/g 比(6.87±0.71)CFU/g、(7.02±0.86)CFU/g]、IL-6[(25.62±5.39)ng/L 比(9.82±2.56)ng/L、(16.59±4.63)ng/L]及IL-8 水平[(62.57±13.68)ng/L 比(29.68±6.73)ng/L、(38.93±10.25)ng/L]高于对照组和IBS-C 组,乳杆菌[(5.73±0.87)CFU/g 比(8.21±1.28)CFU/g、(6.96±0.98)CFU/g]及双歧杆菌含量[(5.96±0.97)CFU/g比(7.83±1.31)CFU/g、(7.02±1.15)CFU/g]、IL-10水平[(18.26±3.85)ng/L比(40.26±8.43)ng/L、(36.71±7.29)ng/L]低于对照组和IBS-C 组(P<0.05),IBS-D 组血清Zonulin 水平高于对照组[(66.78±11.92)μg/L 比(44.13±7.26)μg/L,P<0.05];IBS-C组血清Zonulin、MCP-1、IL-6、IL-8水平,肠杆菌含量高于对照组,乳杆菌、双歧杆菌含量低于对照组(P<0.05);IBS-C组和IBS-D组IBS-SSS评分差异无统计学意义(P>0.05);Pearson相关性分析显示,IBS-C组中血清Zonu?lin水平与乳杆菌、双歧杆菌含量呈负相关(P<0.05);IBS-D组中血清Zonulin水平与乳杆菌、双歧杆菌含量呈负相关(P<0.05),与肠杆菌含量、MCP-1水平、IBS-SSS评分呈正相关(P<0.05),MCP-1水平与乳杆菌及双歧杆菌含量、IL-10水平呈负相关(P<0.05),与IL-6及IL-8水平、IBS-SSS评分呈正相关(P<0.05)。结论IBS病人血清Zonulin、MCP-1水平升高,尤其对于IBS-D病人,血清Zonulin、MCP-1水平与肠道菌群、炎症反应及胃肠症状严重程度具有一定的相关性,有望成为IBS诊断和治疗的辅助指标。

      Objective To investigate the correlation between the serum zonulin (Zonulin) and monocyte chemoattractant protein-1(MCP-1) levels in patients with irritable bowel syndrome (IBS) and the intestinal flora, inflammatory reaction and severity of gastrointes?tinal symptoms.Methods Thirty-five cases of IBS constipation type (IBS-C) patients, 57 cases of IBS diarrhea type (IBS-D) patients,and 50 healthy examiners in The Third People's Hospital of Yunnan from February 2017 to March 2020 were selected as the research objects. The serum Zonulin, MCP-1, interleukin-6 (IL-6), interleukin-8 (IL-8) and interleukin-10 (IL-10) levels were detected, the intes?tinal Bifidobacterium, Lactobacillus, Enterobacter and Enterococcus were cultured, and the bacterial content was calculated. IBS clini?cal symptom severity score system (IBS-SSS) was used to evaluate the severity of IBS. The correlation between zonulin, MCP-1 and in?testinal flora, inflammatory factors and severity of gastrointestinal symptoms were analyzed. Results The serum MCP-1 levels[(313.76±77.93) ng/L vs. (186.52±33.56) ng/L, (268.52±52.65) ng/L], Enterobacter [(9.36±0.81) CFU/g vs. (7.42±0.83) CFU/g, (8.18±0.92) CFU/g] and Enterococcus contents [(8.23±1.21) CFU/g vs. (6.87±0.71) CFU/g, (7.02±0.86) CFU/g], IL-6 [(25.62±5.39) ng/L vs.(9.82±2.56) ng/L, (16.59±4.63) ng/L] and IL-8 levels [(62.57±13.68) ng/L vs. (29.68±6.73) ng/L, (38.93±10.25) ng/L] in IBS-D group were higher than those in the control group and IBS-C group, the Lactobacillus [(5.73±0.87) CFU/g vs. (8.21±1.28) CFU/g, (6.96±0.98)CFU/g] and Bifidobacterium contents [(5.96±0.97) CFU/g vs. (7.83±1.31) CFU/g, (7.02±1.15) CFU/g] and IL-10 level [(18.26±3.85) ng/L vs. (40.26±8.43) ng/L, (36.71±7.29) ng/L] in IBS-D group were lower than those in the control group and IBS-C group (P<0.05), the serum Zonulin level in IBS-D group was higher than that in the control group [(66.78±11.92) μg/L vs. (44.13±7.26) μg/L, P<0.05]. The serum Zonulin, MCP-1, IL-6, IL-8 levels and enterobacteria content in the IBS-C group were higher than those in control group, Lacto?bacillus and Bifidobacterium contents were lower than those in the control group (P<0.05). There was no significant difference in IBSSSS score between the IBS-C group and the IBS-D group (P>0.05). Spearman correlation analysis showed that serum Zonulin level in IBS-C group was negatively correlated with Lactobacillus and Bifidobacterium contents (P<0.05). Serum Zonulin level in IBS-D group was negatively correlated with Lactobacillus and Bifidobacterium (P<0.05), it was positively correlated with Enterobacte content, MCP-1 level, IBS-SSS score (P<0.05), MCP-1 level was negatively correlated with Lactobacillus, Bifidobacterium content, IL-10 level (P<0.05), and it was positively correlated IL-6, IL-8 and IBS-SSS scores (P<0.05).Conclusion The serum Zonulin and MCP-1 levels are elevated in IBS patients. Especially for IBS-D patients, serum Zonulin and MCP-1 levels are correlated with intestinal flora, inflammato?ry reaction, and severity of gastrointestinal symptoms, and are expected to be the biological indicators of diagnosis and therapeutic of IBS.
