| 目的探究壮骨活血汤对骨性关节炎病人血清碱性成纤维细胞生长因子(bFGF)、中性粒细胞巨噬细胞集落刺激因子(GM-CSF)表达及Lysholm膝关节评分的影响。方法选择2019年1月至2020年1月河南省洛阳正骨医院收治的骨性关节炎病人75例，依据随机数字表法将其分成对照组37例与观察组38例，对照组予以“基础治疗+布洛芬缓释胶囊”，观察组在对照组基础上服用壮骨活血汤，疗程4周。比较两组疗效、血清bFGF与GM-CSF表达水平、Lysholm膝关节评分、骨代谢指标。结果观察组总有效率高于对照组，差异有统计学意义(92.11%比72.97%，P<0.05)。治疗后两组血清bFGF、GM-CSF表达水平均低于治疗前(P<0.05)，且治疗后观察组血清bFGF［(1.38±0.33)ng/L比(2.43±0.62)ng/L］、GM-CSF［(2.06±0.65)ng/L比(3.35±1.07)ng/L］表达水平均低于对照组(P<0.05)。治疗后两组Lysholm 膝关节得分均大于治疗前(P<0.05)，且治疗后观察组Lysholm膝关节评分大于对照组［(90.62±3.25)分比(77.84±2.16)分，P<0.05］。治疗后两组Ⅰ型胶原羧基端前肽(PICP)、甲状旁腺素(PTH)、25-羟基维生素D［25-(OH)D］水平均高于治疗前(P<0.05)，且治疗后观察组PICP、PTH、25-(OH)D水平均高于对照组(P<0.05)。结论壮骨活血汤治疗骨性关节炎疗效更佳，能改善膝关节功能，有效调节血清bFGF与GM-CSF表达水平、骨代谢指标。 |

| Objective To explore the effect of Zhuanggu Huoxue decoction on the expressions of serum basic fibroblast growth fac?tor (bFGF), neutrophil macrophage colony stimulating factor (GM-CSF) and Lysholm knee joint score in patients with osteoarthritis.Methods A total of 75 patients with osteoarthritis admitted to Luoyang Orthopedic-Traumatological Hospital of Henan Province from January 2019 to January 2020 were selected.They were divided into control group (n=37) and study group (n=38) according to the ran?dom number table. The control group was given "basic treatment+ibuprofen sustained-release capsule", and the study group was given"Zhuanggu Huoxue decoction" on the basis of the control group for 4 weeks. The therapeutic effect, serum bFGF and GM-CSF expres?sion levels, Lysholm knee joint score and bone metabolism index were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group, and the difference between the two groups was statistically significant (92.11% vs. 72.97%,P<0.05). After treatment, the expression levels of serum bFGF and GM-CSF in the two groups were lower than those before treatment (P<0.05), and the expression levels of serum bFGF [(1.38±0.33) ng/L vs. (2.43±0.62) ng/L] and GM-CSF [(2.06±0.65) ng/L vs. (3.35±1.07) ng/L] in the study group were lower than those in the control group (P<0.05). After treatment, the Lysholm knee score in the two groups were higher than those before treatment (P<0.05), and the Lysholm knee score in the study group was high?er than that in the control group [(90.62±3.25) vs. (77.84±2.16),P<0.05]. After treatment, the levels of carboxyl-terminal propeptide of typeⅠprocollagen (PICP), parathyroid hormone (PTH) and 25-hydroxy vitamin D [25-(OH)D] in the two groups were higher than those before treatment (P<0.05), and the levels of PICP, PTH and 25-(OH)D in the study group were higher than those in the control group (P<0.05).Conclusions The therapeutic effect of Zhuanggu Huoxue decoction on osteoarthritis is better. It can improve the function of knee joint,regulate the expression levels of bFGF and GM-CSF in serum and the index of bone metabolism. |
