      目的探讨U形气道管理在预防有创机械通气病人呼吸机相关性肺炎(VAP)发生的应用价值。方法选取安徽省肿瘤医院2019年1月至2021年12月收治的采用U形气道管理的82例有创机械通气病人作为试验组,同期收治的采用常规气道管理的85例有创机械通气病人为对照组,比较两组病人平均机械通气时间、气道堵塞发生率、气囊压力合格率和床头抬高合格率、VAP发生率。结果试验组病人的气道堵塞发生率、VAP发生率[0.19‰(1/523)比1.53‰(8/524)]低于对照组(均P<0.05);试验组病人的气囊压力合格率、床头抬高合格率均明显高于对照组(P<0.001)。试验组病人急性生理学和慢性健康状况评价Ⅱ(APACHE Ⅱ)评分、平均机械通气时间与对照组病人相比差异无统计学意义(P>0.05)。结论U形气道管理用于有创机械通气病人,病人的气道堵塞等并发症明显减少,可有效降低VAP的发生率。

      Objective To explore the application value of U-shaped airway management in preventing ventilator-associated pneumonia (VAP) in patients on invasive mechanical ventilation.Methods A total of 82 patients with invasive mechanical ventilation who were treated with U-shaped airway management in Anhui Provincial Cancer Hospital from January 2019 to December 2021 were selected as the experimental group, and 85 patients with invasive mechanical ventilation who were treated with conventional airway management during the same period were selected. As the control group, average mechanical ventilation time, incidence of airway obstruction,qualified rate of air bag pressure, qualified rate of bed head elevation, and incidence of VAP were compared between the two groups.Re1sults The incidence of airway obstruction and VAP [0.19‰(1/523) vs. 1.53‰(8/524)] in the experimental group was lower than that in the control group (both P < 0.05); the qualified rates of airbag pressure and bed head elevation in the experimental group were higher than those in the control group (P < 0.001). There was no significant difference in the acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ) score or average mechanical ventilation time between the experimental group and the control group (P > 0.05).Conclu1 sion U-shaped airway management is used for patients with invasive mechanical ventilation, and complications such as airway obstruction are significantly reduced, which can effectively reduce the incidence of VAP.
