      目的探究肝硬化病人血清中微 RNA(microRNA,miR)-454和无翅型小鼠乳腺肿瘤病毒整合位点家族成员 10a(Wnt10a)的表达及其临床意义。方法以 2018年 2月至 2020年 12月入住武汉科技大学附属汉阳医院的 96例肝硬化病人为肝硬化组,并根据 Child-Pugh分级分为 A级 36例、 B级 31例、 C级 29例。另选 60例健康体检者为对照组。抽取空腹外周血检测天冬氨酸转氨酶( AST)、白蛋白( ALB)、丙氨酸转氨酶( ALT)等肝功能指标, qRT-PCR法检测血清中 miR-454和 Wnt10a表达水平,并分析 miR-454、Wnt10a与 Child-Pugh分级评分及 AST、ALT、ALB等肝功能指标的关系;受试者操作特征( ROC)曲线分析 miR-454、Wnt10a对严重肝硬化的诊断效能。结果肝硬化组 miR-454表达水平 0.85±0.20)显著低于对照组 1.46±0.29, Wnt10a表达水平 1.67±0.31显著高于对照组 0.94±0.23(P<0.05);肝硬化组 GGT、CLO、AST、ALP、ALT、TBil水平明显高于对照组,而 ALB水平低于对照组( P<0.05); Child-Pugh A级、 B级、 C级中 miR-454表达水平依次降低( P<0.05); Wnt10a在 Child-Pugh A级、 B级、 C级中表达水平逐渐增加( P<0.05);血清 miR-454与 Wnt10a表达水平呈负相关( r=-0.53,P<0.001);血清 miR-454表达水平与 ALB呈正相关( r>0,P<0.05),与 GLO、TBil、AST、GGT、Child-Pugh分级、 ALT、ALP呈负相关( r<0,P<0.05);血清 Wnt10a表达水平与 ALB呈负相关( r<0,P<0.05)与 GLO、TBil、AST、GGT、Child-Pugh分级、 ALT、ALP呈正相关( r>0,P<0.05); miR-454、Wnt10a诊断严重性肝硬化的曲线下面积分,别为 0.80[95%CI:(0.71,0.88)]、 0.73[95%CI:(0.621,0.837)]; miR-454与 Wnt10a联合诊断的曲线下面积为 0.84[95%CI:(0.75,0.93)]。结论肝硬化病人血清中 miR-454下调表达, Wnt10a上调表达, miR-454和 Wnt10a呈负相关性,二者均与肝功能损伤有关,有作为诊断肝硬化严重程度的潜能。

      Objective To investigate the expression and clinical significance of microRNA (miR)-454 and wingless-type mouse mam-mary tumor virus integration site family, member 10A (Wnt10a) in the serum of patients with liver cirrhosis and its clinical significance.Methods A total of 96 patients with cirrhosis admitted to Hanyang Hospital Affiliated to Wuhan University of Science and Technolo-gy from February 2018 to December 2020 were selected as the cirrhosis group, and they were assigned into 36 cases in grade A, 31 cas-es in grade B, and 29 cases in grade C according to the Child-Pugh grading. Another 60 healthy subjects were selected as the controlgroup. The fasting peripheral blood was taken to detect liver function indicators such as aspartate aminotransferase (AST), albumin(ALB), alanine aminotransferase (ALT), etc. The expression levels of miR-454 and Wnt10a in serum were detected with qRT-PCR method. The relationship between miR-454, Wnt10a and Child-Pugh grading score and liver function indicators, such as AST, ALT, ALB, was analyzed; the diagnostic efficacy of miR-454 and Wnt10a for severe liver cirrhosis was analyzed with ROC curve. Results The expression level of miR-454 in the cirrhosis group 0.85±0.20 was significantly lower than that in the control group 1.46±0.29, andthe expression level of Wnt10a 1.67±0.31 was significantly higher than that in the control group 0.94±0.23 (P<0.05); the levels of gluta-myltransferase (GGT), globulin (CLO), AST, ALP, ALT, and total bilirubin (TBil) in the liver cirrhosis group were significantly higherthan those in the control group, while the level of ALB was lower than that in the control group (P<0.05); the expression level of miR-454 in Child-Pugh grade A, B, and C decreased sequentially (P<0.05); the expression level of Wnt10a in Child-Pugh grade A, B, and C in- creased gradually (P<0.05); serum miR-454 was negatively correlated with the expression level of Wnt10a (r=-0.53, P<0.001); the ex- pression level of serum miR-454 was positively correlated with ALB (r>0, P<0.05), and negatively correlated with GLO, TBil, AST, GGT, Child-Pugh grading, ALT, ALP (r<0, P<0.05); the expression level of serum Wnt10a was negatively correlated with ALB (r<0, P< 0.05), and positively correlated with GLO, TBil, AST, GGT, Child-Pugh grading, ALT, ALP (r>0, P<0.05); the area under the curve of miR-454 and Wnt10a in the diagnosis of severe liver cirrhosis was 0.80 [95%CI:(0.71,0.88)] and 0.73 [(95%CI: 0.62,0.84)], respectively; the area under the curve of the combined diagnosis of miR-454 and Wnt10a was 0.84 [95%CI:(0.75,0.93)].Conclusions The expres- sion of miR-454 is down-regulated and Wnt10a is up-regulated in patients with liver cirrhosis. There is a negative correlation between miR-454 and Wnt10a. Both of these factors are related to liver damage and have the potential to diagnose the severity of liver cirrhosis.
