      目的探讨核转录因子红系 2相关因子 2(Nrf2)与选择性自噬接头蛋白 p62/Sequestosome1(SQSTM1)在诊断宫颈癌和上皮内病变中的价值,并且分析二者在不同宫颈病变中的相关性。方法收集 2008年 1月至 2021年 12月南通市肿瘤医院 120例宫颈鳞状细胞癌(SCC)102例低级别鳞状上皮内病变(LSIL)和 101例高级别鳞状上皮内病变(HSIL)病人及 49例宫颈良性/反应性鳞状上皮病人的石蜡、标本,免疫组织化学方法检测其中 Nrf2和 p62的表达,并分析二者在不同宫颈病变中的相关性及评估二者在诊断中的价值。结果 Nrf2和 p62在 LSIL、HSIL和 SCC中表达明显高于良性 /反应性宫颈鳞状上皮(均 P<0.05), Nrf2和 p62在 HSIL和 SCC中表达明显高于 LSIL(均 P<0.05),p62在 SCC中表达明显高于 HSIL(P<0.05),而 Nrf2在 HSIL中表达稍低于 SCC(P<0.05)。 Nrf2和 p62在良性 /反应性宫颈鳞状上皮、 LSIL、HSIL和 SCC中均具有正相关性,相关系数分别为 0.63、 0.58、0.69和 0.38(均 P<0.05)。 ROC曲线结果显示,除 Nrf2在诊断 HSIL与 SCC中差异无统计学意义外, Nrf2、p62以及联合 Nrf2和 p62在诊断良性 /反应性鳞状上皮与 LSIL、良性 /反应性鳞状上皮与 HSIL、良性 /反应性鳞状上皮与 SCC、LSIL与 HSIL、LSIL与 SCC、HSIL与 SCC各个组别中均差异有统计学意义(均 P<0.05)。结论 Nrf2和 p62在良性 /反应性鳞状上皮中不表达或低表达, LSIL中表达有所增高, HSIL和 SCC中表达最高,二者在不同宫颈病变中均具有正相关性,而且单独使用 Nrf2或 p62就能够有效诊断不同的宫颈病变,二者联用诊断效果更佳。

      Objective To explore the value of nuclear factor-erythroid 2-related factor 2 (Nrf2) and selective autophagy junction proteinp62/Sequencosome1 (SQSTM1) in the diagnosis of cervical cancer and intraepithelial lesions, and to analyze the correlation betweenNrf2 and p62 in different cervical lesions. Methods The paraffin specimens of 120 cases with cervical squamous cell carcinoma (SCC), 102 cases with low-grade squamous intraepithelial lesions (LSIL), 101 cases with high-grade squamous intraepithelial lesions(HSIL), and 49 cases with benign/reactive squamous epithelium were collected in Nantong Tumor Hospital from January 2018 to De.cember 2021. The expression levels of Nrf2 and p62 were evaluated through immunohistochemical staining. The correlation betweenNrf2 and p62 and their value in diagnosis was also analyzed.Results The expression of Nrf2 and p62 in LSIL, HSIL and SCC was sig.nificantly higher than that in benign/reactive cervical squamous epithelium (P<0.05), the expression of Nrf2 and p62 in HSIL and SCC was also significantly higher than that in LSIL (P<0.05). The expression of p62 in SCC was significantly higher than that in HSIL (P< 0.05), while the expression of Nrf2 in HSIL was slightly lower than that in SCC (P<0.05); Nrf2 and p62 were positively correlated in be.nign/reactive cervical squamous epithelium, LSIL, HSIL and SCC and their correlation coefficients were 0.63, 0.58, 0.69 and 0.38, re.spectively (P<0.05); ROC curve showed that Nrf2 had no statistical significance in the diagnosis of HSIL and SCC, while Nrf2, p62 and the combination of Nrf2 and p62 were statistically significant in the diagnosis of cervical benign/reactive squamous epithelium andLSIL, cervical benign/reactive squamous epithelium and HSIL, cervical benign/reactive squamous epithelium and SCC, LSIL andHSIL, LSIL and SCC, HSIL and SCC (P<0.05).Conclusions Nrf2 and p62 are not expressed or poorly expressed in benign/reactivesquamous epithelium, the expression levels of Nrf2 and p62 are increased in LSIL, and highest in HSIL and SCC. Nrf2 is positively cor.related with p62 in different cervical lesions. Moreover, Nrf2 or p62 alone can effectively diagnose different cervical lesions, and thecombination of these two markers can be better effective than alone.
