| 目的探讨自拟外洗方治疗尿毒症性皮肤瘙痒症( UP)病人的疗效。方法选取苏州市中西医结合医院 2019年 4月至 2020年 4月收治的 UP病人 86例，按随机数字表法分为观察组和对照组，每组各 43例。两组均采取常规透析疗法，对照组采用瘙痒部位外涂炉甘石洗剂治疗，观察组采用自拟外洗方进行药浴，疗程 4周。比较两组治疗前与治疗 4周、随访至 12周的瘙痒程度［采用四项目瘙痒量表( FIIQ)评估］、生活质量［采用生活质量综合评定问卷 -74(GQOL-74)评估］与睡眠质量［采用匹兹堡睡眠质量指数( PSQI)评估］及血钙、血磷、全段甲状旁腺激素( iPTH)、 C反应蛋白( CRP)、白细胞介素 -31(IL-31)、血 β2-微球蛋白( β2-MG)水平。结果对照组和观察组治疗前的 FIIQ评分为( 11.23±1.89)分和( 11.07±1.72)分，治疗 4周分别降低至( 6.17±1.58)分和( 4.68±1.11)分，随访至 12周为( 6.54±1.64)分和( 4.77±1.36)分，组间比较、时间点比较及交互作用的差异有统计学意(P<0.001)。两组病人 GQOL-74评分、 PSQI评分、血磷、 iPTH、血清 CRP、IL-31、β2-MG的组间比较、时间点比较及交互作用的差义异有统计学意义( P<0.001)。血钙的组间比较、时间点比较及交互作用的差异无统计学意义( P>0.05)。结论自拟外洗方治疗 UP可有效缓解病人瘙痒症状，降低血清 CRP、IL-31、β2-MG水平，提高睡眠质量与生活质量。 |

| Objective To investigate the therapeutic effect of self-designed external washing prescription on uremic pruritus (UP). Methods A total of 86 patients with UP admitted to Suzhou Hospital of Integrated Traditional Chinese and Western Medicine fromApril 2019 to April 2020 were selected and divided into observation group and control group by random number table method, with 43cases in each group. The two groups were treated with routine dialysis therapy, the control group was treated with calamine lotion ap.plied to the pruritus, and the observation group was treated with self-designed prescription for external washing. The course of treatment was 4 weeks. The degree of pruritus [measured by four-item Itch Scale (FIIQ)], quality of life [measured by Generic Quality of Life In. ventory-74(GQOL-74)], sleep quality [measured by Pittsburgh Sleep Quality Index (PSQI)], blood calcium, blood phosphorus and intact parathyroid hormone (iPTH), C-reactive protein (CRP), interleukin-31 (IL-31), and blood β2-microglobulin (β2-MG) levels after 4 weeks of treatment and follow-up to 12 weeks were compared between the two groups.Results The FIIQ scores before treatment in thecontrol group and the observation group were (11.23±1.89) points and (11.07±1.72) points, which decreased to (6.17±1.58) points and(4.68±1.11) points at 4 weeks of treatment, respectively, and were (6.54±1.64) points and (4.77±1.36) points at 12 weeks of follow-up, and there were significant differences between groups, time point comparison and interaction (P<0.001). There were significant differ. ences in GQOL-74 score, PSQI score, blood phosphorus, iPTH, serum CRP, IL-31, β2-MG between groups, time point comparison and interaction (P<0.001). There were no significant difference between groups, time point comparison and interaction of blood calcium (P> 0.05).Conclusion The self-designed external washing formula can effectively relieve pruritus symptoms in patients with UP, reduce serum CRP, IL-31, β2-MG levels, and improve sleep quality and quality of life. |
