| 目的 探讨细胞因子在骨髓增殖性肿瘤(MPN)病人外周血中的水平及其意义。方法 采用免疫荧光法检测2013年5月至2017年2月安庆市立医院血液科收治的68例MPN病人,包括真性红细胞增多症(PV)22例,原发性血小板增多症(ET)16例,原发性骨髓纤维化(PMF)11例,慢性粒细胞白血病(CML)19例及20例健康对照者。检测外周血中白介素(IL)-1β、IL-2R、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)及IL-10表达水平。结果 PMF组中,IL-2R、IL-6、IL-8及IL-10水平较对照组明显升高,均P＜0.05；PV及ET组中,IL-2R及IL-8较对照组明显升高,均P＜0.05；CML组中,IL-2R、IL-6及IL-8水平较对照组明显升高,均P＜0.05。与ET组和PV组相比,PMF组表现出更高的IL-2R、IL-6和IL-10浓度,分别为(3 769.32±25.46)、(7.40±1.42)、(16.55±3.13) ng/L。与ET和PV相比,CML组IL-6的浓度也更高[(7.21±1.35) ng/L]。与CML组相比,PMF病人表现出更高浓度的IL-2R[(3 769.32±25.46)ng/L]。PV组IL-2R与血红蛋白(HGB)浓度呈正相关；ET组IL-2R与白细胞、血小板数值均呈正相关；PMF组IL-2R与白细胞计数、红细胞输注次数及JAK2V617突变均呈正相关。结论 MPN病人中细胞因子的表达出现失调,可能是导致MPN 发生、发展的重要因素之一。 |

| Objective To investigate the level and significance of cytokines in peripheral blood of patients with myeloproliferative neoplasm (MPN).Methods The levels of interleukin (IL)-1β,IL-2R,IL-6,IL-8,tumor necrosis Factor-α (TNF-α) and IL-10 in peripheral blood of 68 patients with MPN admitted to the Department of Hematology,Anqing Municipal Hospital from May 2013 to February 2017 and 20 healthy controls were detected by immunofluorescence.MPN patients included 22 cases of polycythemia vera (PV),16 cases of essential thrombocythemia (ET),11 cases of primary myelofibrosis (PMF) and 19 cases of chronic myeloid leukemia (CML).Results The levels of IL-2R,IL-6,IL-8 and IL-10 in the PMF group were significantly higher than those in the control group (all P<0.05).In the PV and ET groups,IL-2R and IL-8 levels were significantly higher than those in the control group (all P<0.05).The levels of IL-2R,IL-6 and IL-8 were significantly higher in the CML group than in the control group (all P<0.05).Compared with the ET group and the PV group,the PMF group showed higher concentrations of IL-2R,IL-6 and IL-10,respectively,with(3 769.32±25.46),(7.40±1.42),(16.55±3.13) ng/L.The concentration of IL-6 in the CML group was also higher [(7.21±1.35) ng/L] than those in the ET and PV groups.PMF patients showed higher concentrations of IL-2R (3 769.32±25.46) ng/L compared to CML group.There was a positive correlation between IL-2R level and hemoglobin(HGB) concentration in PV group.IL-2R level in ET group was positively correlated with white blood cell and platelet count.The level of IL-2R in PMF group was positively correlated with white blood cell count,red blood cell infusion and JAK2V617 mutation.Conclusions The expression of cytokines in patients with MPN was maladjusted,which may be one of the important factors that lead to the occurrence and development of MPN. |
