| 目的 评价氟伏沙明与无抽搐电休克(MECT)联合不同剂量阿立哌唑治疗强迫症的疗效及安全性。方法 选取2012年1月至2017年1月濮阳市精神卫生中心收治的强迫症病人200例,根据随机数字表法将其分为A、B、C、D、E五组,每组40例。A组病人使用氟伏沙明和MECT治疗,B、C、D、E组病人给予氟伏沙明和MECT治疗的同时,分别给予不同剂量的阿立哌唑治疗(剂量分别为2.5、5.0、7.5、10.0 mg/d)。12周后对其临床效果及安全性进行评价。结果 治疗后各组耶鲁布朗强迫量表(Y-BOCS)评分明显下降、生存质量综合评定(GQOLI-74)评分明显升高,与治疗前比较,均差异有统计学意义(P<0.05)；A组、B组、C组Y-BOCS评分、GQOLI-74评分比较,差异有统计学意义(P<0.05)； A组、B组、C组、D组、E 组总有效率分别为62.50%、87.50%、90.00%、65.00%、57.50%,差异有统计学意义(χ2=18.307,P=0.001)；五组症状量表(TESS)评分、不良反应发生率比较,差异无统计学意义(P>0.05)。结论 氟伏沙明与MECT联合小剂量(2.5、5.0 mg/d)阿立哌唑治疗强迫症效果显著,且较安全。 |

| Objective To evaluate the effects of Fluvoxamine and modified electroconvulsive therapy (MECT) efficacy and safety when combined with different doses of Aripiprazole in the treatment of obsessive-compulsive disorder.Methods Two hundred patients admitted to Puyang Mental Health Center from January 2012 to January 2017 were selected.According to the randomized digital table method,they were randomly assigned into 5 groups:A,B,C,D,E,with 40 cases in each group.Group Awas treated with Fluvoxamine and MECT,while groups B,C,D,and Ewere treated with Fluvoxamine and MECT,at the same time,Aripiprazole was used at different doses (2.5,5.0,7.5,0.0 mg/d,respectively).After 12 weeks,the clinical effect and safety were evaluated.Results The scores of Yale-Brown Obsessive Compulsive Scale (Y-BOCS) in each group decreased obviously after treatment,quality of life synthetic score (GQOLI-74) was significantly higher,compared with before treatment;the differences were statistically significant (all P<0.05);Y-BOCS and GQOLI-74 scores of group A,group Band group Cwere compared,and the differences were statistically significant (all P<0.05);the total effective rates of group A,group B,group C,group Dand group Ewere 62.50%,87.50%,90%,65%,57.50%,respectively,and the difference was statistically significant (χ2=18.307,P=0.001);symptom scale (TESS) score,the incidence of adverse reactions of the 5 groups were compared,and the differences were not statistically significant (all P>0.05).Conclusions Fluvoxamine and MECT combined with small dosage (2.5 mg/d and 5.0 mg/d) of Aripiprazole in the treatment of obsessive-compulsive disorder is effective and safer. |
