目的探讨生长抑制特异性转录本 5(GAS5)和微小 RNA-142-3p(miR-142-3p)在重症急性胰腺炎( SAP)合并急性呼吸窘迫综合征( ARDS)病人血清中的表达及临床意义。方法选取 2017年 2月至 2018年 12月平顶山市第一人民医院 SAP病人 83例,采用随机数字表法抽样 41例并发 ARDS者为 ARDS组, 42例非 ARDS组。另选取同时期 40例健康体检者作为健康对照组。实时荧光定量 PCR(RT-qPCR)检测病人血清 GAS5和 miR-142-3p表达水平, Pearson相关性分析 SAP合并 ARDS病人血清 GAS5与 miR-142-3p表达相关性,受试者工作特征( ROC)曲线分析血清 GAS5和 miR-142-3p对 SAP合并 ARDS的诊断价值, logistic回归分析 SAP合并 ARDS及其预后的影响因素。结果与健康对照组比较,非 ARDS组和 ARDS组病人血清 GAS5表达升高( P<0.05)miR-142-3p表达水平降低( P<0.05)。与非 ARDS组比较, ARDS组病人血清 GAS5表达升高( P<0.05),miR-1423p表达水平降低,(P<0.05)。 Pearson相关性分析结果显示, SAP合并 ARDS病人血清 GAS5与 miR-142-3p呈负相关(P<0.05)。 GAS5与 miR-142-3p联合检测诊断 SAP合并 ARDS的灵敏度、特异度及曲线下面积( AUC)均高于 GAS5、miR-142-3p单独诊断(P<0.05)。与 SAP合并 ARDS存活病人比较,死亡病人血清 GAS5表达水平升高(P<0.05),miR-142-3p表达水平降低( P<0.05)。 GAS5和 miR-142-3p是 SAP病人合并 ARDS的显著影响因素( P<0.05),也是 SAP合并 ARDS病人预后的显著影响因素( P<0.05)。结论 SAP合并 ARDS病人血清中 GAS5表达升高, miR-142-3p表达降低,可能为 SAP合并 ARDS的诊断和预后判断提供了新的生物学指标。 |

Objective to investigate the expression and clinical significance of growth suppression-specific transcription 5 (GAS5) and micrna-142-3p (mir-142-3p) in the serum of patients with severe acute pancreatitis (SAP) combined with acute respiratory distress syndrome (ARDS).Methods A total of 41 patients with ARDS were randomly sampled from SAP patients in Pingdingshan First People′s Hospital from February 2017 to December 2018, including the ARDS group and the non-ards group. In addition, 40 healthy physical examinees were selected as the healthy control group. Real-time fluorescence quantitative PCR (rt-qpcr) was used to detect the expression levels of serum GAS5 and mir-142-3p in patients with SAP combined with ARDS. Pearson correlation was used to analyze thecorrelation between the expression levels of serum GAS5 and mir-142-3p in patients with SAP combined with ARDS. ROC curve was used to analyze the diagnostic value of serum GAS5 and mir-142-3p in patients with SAP combined with ARDS.Results Compared with the healthy control group, the expression of serum GAS5 in the non-ARDS group and ARDS group increased (P<0.05), and the expression level of miR-142-3p decreased (P<0.05), and the expression level of mir-142-3p was decreased (P<0.05). Compared with the non-ards group, the expression of serum GAS5 in ARDS group was increased (P<0.05), and the expression level of mir-142-3p was decreased (P<0.05). Pearson correlation analysis results showed that the serum GAS5 of SAP patients with ARDS was negatively correlated with mir-142-3p (P<0.05). The sensitivity, specificity and AUC of both GAS5 and mir-142-3p were higher than that of GAS5 and mir-142-3p alone (P<0.05). Compared with the surviving patients with SAP combined with ARDS, the expression level of serum GAS5in the deceased patients was increased (P<0.05), and the expression level of mir-142-3p was decreased (P<0.05). GAS5 and mir-1423p were significant factors affecting ARDS in SAP patients combined with ARDS (P <0.05) and the prognosis of SAP patients combined with ARDS (P <0.05).Conclusion The expression of GAS5 in the serum of patients with SAP combined with ARDS is increased, and the expression of miR-142-3p is decreased, which may provide new biological indicators for the diagnosis and prognosis of SAP combined with ARDS. |
