      目的观察肺结核病人外周血中长链非编码 RNA MIR155宿主基因( LncRNAMIR155HG)的表达,研究与 T细胞的关系,探讨在疾病发生中的意义。方法收集 2018年 1月至 2019年 1月河南科技大学第一附属医院就诊的肺结核(肺结核组)治疗病人 126例,同时取 150例体检健康者作为对照组。实时荧光定量 PCR(qRT-PCR)检测外周血中 LncRNA MIR155HG水平,酶联免疫吸附( ELISA)法检测外周血中 T细胞指标白介素( IL)-2、干扰素( IFN-γ)、 IL-4、IL-5、辅助性 T细胞 17A(Th-17A)水平; Pearson法分析肺结核病人外周血中 LncRNA MIR155HG与 T细胞指标相关性; logistic分析影响肺结核的因素;受试者工作特性曲线( ROC)分析外周血中 LncRNA MIR155HG对肺结核的诊断价值。结果肺结核组和对照组外周血中 LncRNA MIR155HG相对表达量 OD值分别为( 1.96±0.62)(1.02±0.26),与对照组相比,肺结核组外周血中 LncRNA MIR155HG、IL-4、IL5、Th-17A水平升高( P<0.05)IL-2、IFN-γ降低( P<0.05)。 Pearson法分析结果显示,肺结核病人外周血中 LncRNA MIR155HG与 IL-2、IFN-γ呈负相关( P<0.05),与 IL-4、Th-17A呈正相关( P<0.05)。 logistic回归分析显示, LncRNA MIR155HG、IL-4是影响肺结核的危险因素, IFN-γ是影响肺,结核的保护因素。 ROC曲线显示,外周血中 LncRNA MIR155HG水平预测肺结核的 ROC为 0.920(95%CI:0.880~0.960),截断值为 1.358,其敏感性为 87.3%、特异性为 99.2%。结论肺结核病人外周血 LncRNA MIR155HG中高表达,与 T细胞指标关系密切,可在一定程度上诊断肺结核,可为早期诊断肺结核提供一定的依据。

      Objective To observe the expression of long noncoding RNA (LncRNA) MIR 155HG in peripheral blood of patientswith pulmonary tuberculosis, to study its relationship with T cells, and to explore its significance in the pathogenesis of pulmonary tuberculosis.Methods A total of 126 patients with pulmonary tuberculosis (tuberculosis group) treated in The First Affiliated Hospitalof Henan University of Science and Technology from January 2018 to January 2019 were collected, and 150 healthy persons were takenas control group. Real-time fluorescence quantitative PCR (qRT-PCR) was used to detect the level of LncRNA MIR155HG in peripheral blood, enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of interleukin (IL) -2, interferon (IFN-γ), IL-4, IL-5 and Th-17A in peripheral blood; Pearson method was used to analyze the correlation between LncRNA MIR155HG and T cell index inperipheral blood of patients with pulmonary tuberculosis; Logistic analysis was used to analyzed the factors affecting tuberculosis; andthe diagnostic value of LncRNA MIR 155HG in peripheral blood for tuberculosis was analyzed by ROC.Results The OD value of the relative expression of LncRNA MIR155HG in the peripheral blood of the tuberculosis group and the control group were 1.96±0.62,1.02±0.26, respectively. Compared with those in the control group, the levels of LncRNA MIR 155HG, IL-4, IL-5 and Th-17A in peripheral blood of pulmonary tuberculosis group increased (P < 0.05), while the levels of IL-2 and IFN-γ decreased (P < 0.05). Pearsonanalysis showed that LncRNA MIR155HG in peripheral blood of patients with pulmonary tuberculosis was negatively correlated withIL-2 and IFN-γ(P < 0.05), and positively correlated with IL-4 and Th-17A (P < 0.05). Logistic regression analysis showed that LncRNA MIR155HG and IL-4 were risk factors for tuberculosis, and IFN-γ was a protective factor for tuberculosis. The ROC curveshowed that the area under the ROC curve predicted by the level of LncRNA MIR 155HG in peripheral blood was 0.920 (95% CI:0.880-0.960), the truncation value was 1.358, and the sensitivity and specificity were 87.3% and 99.2%. Conclusions LncRNA MIR155HG is highly expressed in peripheral blood of patients with pulmonary tuberculosis and is closely related to T cell markers,which can diagnose pulmonary tuberculosis to some extent and provide a basis for early diagnosis of pulmonary tuberculosis.
