| 目的 研究自拟温阳解毒消瘿方和小剂量左甲状腺素片(LST)对高碘地区结节性甲状腺肿(NG)患者的疗效。方法 选取2016年5月至2017年3月黄骅市人民医院接受救治的200例NG患者作为研究对象。按随机数字表法分成观察组和对照组,每组100例。对照组给予单纯的LST治疗,观察组给予自拟温阳解毒消瘿方联合小剂量左甲状腺素片治疗,对比观察两组患者治疗前后的临床疗效、甲状腺肿症状与甲状腺最大直径肿物、甲状腺激素水平变化及甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)等指标。结果 观察组痊愈率12.00%(12/100),总有效率89.00%(89/100),显著高于对照组4.00%(4/100),77.00%(77/100),差异有统计学意义(P＜0.05)；甲状腺肿症状与甲状腺肿物的最大直径在治疗前差异无统计学意义,治疗后均显著下降(均P＜0.05)；治疗前血清游离三碘甲腺原氨酸(FT3)、血清游离甲状腺素(FT4)及促甲状腺激素(TSH)各项指标均差异无统计学意义,治疗后FT3、FT4指标均显著升高,且TSH指标显著降低,差异有统计学意义(均P＜0.05)；治疗前两组患者TPOAb及TGAb指标均差异无统计学意义,治疗后两组指标均显著降低(均P＜0.05)。结论 自拟温阳解毒消瘿方联合小剂量左甲状腺素片治疗高碘地区结节性甲状腺肿的临床研究效果显著,对有效控制甲状腺肿症状与甲状腺最大直径肿物,改善患者FT3、FT4、TSH及TPOAb和TGAb指标效果明显,同时还具有相对优越的安全性,值得推广。 |

| Objective To study the effect of Wenyang Jiedu decoction combined with low-dose levothyroxin sodium tablets (LST) in the treatment of nodular goiter (NG) in iodine excess area.Methods A total of 200 cases of NG patients who were treated in Huanghua People's Hospital from May 2016 to March 2017 were selected as the research subjects.The patients were randomly assigned into observation group (100 cases) and control group (100 cases).The control group was given LST treatment alone,while the observation group was given Wenyang Jiedu decoction combined with small-dose LST.Observation of the indicators was made in two groups of patients before and after treatment,including clinical efficacy,symptoms of goiter,thyroid tumor with maximum diameter,change of thyroid hormone levels,thyroglobulin antibodies (TGAb),thyroid peroxidase antibody (TPOAb) and other indicators.Results The recovery rate and the overall effective rate in observation group accounting for 12% (12/100) and 89% (89/100),were significantly higher than 4% (4/100),and 77% (77/100) in control group;the difference was statistically significant (P＜0.05).There were no significant differences in symptoms of goiter and maximum diameter of thyroid tumor between two groups before treatment;after treatment both were significantly decreased (all P＜0.05).Before treatment,there were no statistically significant differences in FT3,FT4 and TSH,after treatment the levels of FT3 and FT4 were significantly increased,and the level of TSH was significantly decreased (all P＜0.05).Before treatment,there were no significant differences in TPOAb and TGAb between the two groups;after treatment,the levels of TPOAb and TGAb were significantly reduced (all P＜0.05).Conclusion The clinical efficacy of Wenyang Jiedu decoction combined with small-dose levothyroxine tablets in the treatment of nodular goiter in iodine excess area is significant,which can effectively control the symptoms of goiter and the maximum diameter of tumor,improve the patients' FT3,FT4,TSH,TPOAb and TGAb levels.Meanwhile,it is relatively safe,which is worthy of promotion. |
