      目的探讨活动期肺结核病人外周血中微小RNA-144(mir-144)的表达水平及临床意义。方法 选择2016年1月至2017年4月哈尔滨医科大学附属第一医院活动期肺结核病人98例,另外选择同期进行体检的健康志愿者62例作为对照组。检测所有受试者血液中(mir-144)和血浆中肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)及单核细胞趋化蛋白-1(MCP-1)表达水平,分析mir-144的临床诊断意义。结果 与活动期肺结核病人相比,对照组血液中mir-144的表达水平明显升高[(1.53±0.35)比(0.78±0.21)],而血浆中TNF-α[(5.03±1.04) pg/mL 比(30.63±7.23) pg/mL]、IL-10[(3.26±0.85) pg/mL比(24.08±5.17) pg/mL]及MCP-1[(201.33±46.87) pg/mL比(252.12±58.63) pg/mL]表达水平显著降低,差异有统计学意义(P<0.05);活动期肺结核病人血液中mir-144的表达水平与血浆中TNF-α、IL-10及MCP-1表达水平呈负相关(P<0.05);绘制血浆mir-144诊断活动性肺结核的受试者工作特征曲线,曲线下面积为0.944,用最大约登指数法确定血浆mir-144诊断活性期肺结核的截断值为1.47,用其诊断活动性肺结核的灵敏度和特异度分别为96.88%和96.77%。结论 活动期肺结核病人外周血中的mir-144的表达水平异常降低,并且与相关指标密切联系,临床诊断意义重大,能够作为临床诊断活动期肺结核的生物指标。

      Objective To investigate the expression and clinical significance of mir-144 in peripheral blood of patients with active pulmonary tuberculosis.Methods Ninety-eight cases of patients with active pulmonary tuberculosis admitted to The First Affiliated Hospital of Harbin Medical University from January 2016 to April 2017 were enrolled in the study.Another 62 healthy volunteers were enrolled as control group over the same period.The expression levels of mir-144 in the blood and tumor necrosis factor-α (TNF-α),interleukin-10 (IL-10) and monocyte chemoattractant protein-1 (MCP-1) in plasma were detected,and the clinical diagnostic significance of mir-144 was analyzed.Results Compared with patients with active pulmonary tuberculosis,the expression of mir-144 of subjects in the control group significantly increased [(1.53±0.35) vs. (0.78±0.21)],while the levels of TNF-α [(5.03±1.04) pg/mL vs. (30.63±7.23) pg/mL],IL-10 [(3.26±0.85) pg/mL vs. (24.08±5.17) pg/mL] and MCP-1 [(201.33±46.87) pg/mL vs. (252.12±58.63) pg/mL] significantly decreased,and the differences were statistically significant (P<0.05).The expression level of mir-144 in the blood of patients with active pulmonary tuberculosis was negatively correlated with the expression levels of TNF-α,IL-10 and MCP-1 in plasma (P<0.05).ROC curves were drawn of plasma mir-144 in the diagnosis of active pulmonary tuberculosis,and the area under the curve was 0.944.With the maximum Youden index method the truncation value of plasma mir-144 in diagnosis of active pulmonary tuberculosis was determined to be 1.47,and the sensitivity and specificity were 96.88% and 96.77%,respectively.Conclusion The expression of mir-144 in the peripheral blood of active pulmonary tuberculosis patients decreases abnormally,and is correlated with the relevant indicators,which has great clinical diagnostic significance,and can be used as a biological indicator for the clinical diagnosis of active pulmonary tuberculosis.
