      目的研究颈动脉血管重建术对眼缺血综合征病人球后血管血流动力学及血清血管内皮细胞生长因子(VEGF)、促红细胞生成素( EPO)水平的影响。方法选取 2017年 11月至 2018年 11月邯郸市中心医院收治的 32例眼缺血综合征病人进行研究,采用随机数字表法将其分为对照组与观察组,各 16例。对照组采用眼科保守治疗;观察组病人在眼科保守治疗的同时接受颈动脉支架形成术( CAS)治疗,比较治疗前、治疗后 1周及治疗后 6个月两组病人的眼动脉及视网膜中央动脉的收缩期血流峰值流速( PSV)、舒张末期血流速度( EDV)及阻力指数( RI);比较治疗前及治疗 6个月后两组病人的血清 VEGF、EPO、白细胞介素 -6(IL-6)水平、眼压和最佳矫正视力。结果与治疗前相比,治疗后 1周,两组病人的 PSV、EDV水平升高, RI水平降低,治疗后 6个月两组病人的血清 VEGF、IL-6及眼压水平降低、最佳矫正视力升高( P<0.05);与治疗后 1周对照组相比,治疗后 1周观察组病人的 PSV、EDV水平较高, RI水平较低( P<0.05);与治疗后 6个月对照组[( 385.78±105.62)ng/L,(22.31±4.27)ng/L,(23.58±4.12)mmHg]相比,治疗后 6个月观察组病人的血清 VEGF、IL-6及眼压水平[分别为( 272.17±81.51)ng/L,(23.42±4.31) ng/L,(17.77±3.21)mmHg]较低( P<0.05)。结论颈动脉血管重建术可改善眼缺血综合征病人球后血管血流动力学降低血清中 VEGF、而对血清中 EPO水平的无影响。

      Objective To study the effects of carotid artery revascularization on the retrobulbar hemodynamics and the levels of serumvascular endothelial growth factor (VEGF)and erythropoietin (EPO) in patients with ocular ischemic syndrome.Methods A total of 32 patients with ocular ischemic syndrome treated in Handan Central Hospital from November 2017 to November 2018 were enrolled in thisstudy and assigned into control group and observation group, with 16 cases in each group. The control group was treated with ophthalmicconservative therapy, and the observation group was treated with carotid angioplasty and stenting (CAS) in addition to ophthalmic conservative treatment. The peak systolic velocity (PSV), end diastolic velocity (EDV) and resistance index (RI) of the ophthalmic artery and thecentral retinal artery were compared before treatment, 1 week and 6 months after treatment; the levels of serum vascular endothelial growthfactor (VEGF), erythropoietin (EPO), interleukin-6 (IL-6), and the intraocular pressure, the best corrected visual acuity were compared between the two groups before and 6 months after treatment.Results Compared with those before treatment, the levels of PSV and EDV inthe two groups increased at one week after treatment, and RI level decreased, the levels of VEGF, IL-6 and intraocular pressure in the two groups at 6 months after treatment decreased, and the best corrected visual acuity increased (P<0.05). Compared with the control group atone week after treatment, the PSV and EDV levels in the observation group were higher, and RI level was lower (P<0.05).
